A DAY IN THE LIFE OF A SPEECH LANGUAGE PATHOLOGIST (SLP)
As an SLP, you will evaluate, plan, and administer medically prescribed or educationally relevant speech therapy treatment to clients in their homes or students in their respective educational environment. In addition, this position is responsible for clinical supervision of students and SLP's in CFY year and students. Responsibilities include but are not limited to the following:
<ul>
<li>Actively monitors and amends patient visit schedule to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc.</li>
<li>Performs a comprehensive (OASIS and SLP) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.</li>
<li>Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.</li>
<li>Plans for and provides skilled speech-language therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.</li>
<li>As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record.</li>
<li>Accurately and completely performs functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork.</li>
<li>As necessary, assesses patients' needs for HCA services, develops and coordinates POC with patient/care giver, and provides on-going supervision.</li>
<li>Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed.</li>
<li>Serve as a Home Care ST as required</li>

Qualifications:

<li>Master's degree in Speech Language Pathology required</li>
<li>Current Vermont State SLP license required</li>

<li>Obtains clinical rational when needed.</li>
<li>Schedules peer to peers with the provider when needed.</li>
<li>Assists with Specialty Pharmacy Prior AUTH's when needed.</li>
<li>Contacts patient to discuss alternative medications as well as the requirements and instructions from their insurance companies.</li>
<li>Contacts Pharmacies to discuss alternative medications as well as the requirements and instructions from the patient's insurance companies.</li>
<li>Assist with answering questions on Tier coverage.</li>
<li>Working knowledge of insurance policies regarding necessary requirements to obtaining a Prior Authorization (I.e., when the PA is needed vs making sure there is 120-day refill of the rescue inhaler).</li>
<li>Assist with scheduling follow-up appointments for patients in none in place at time of call to patient.</li>
</ul>
QUALIFICATIONS:
<ul>
<li>Excellent communication and problem solving skills at a level necessary for understanding provider and supervisors instructions, and for accurately documenting patients' medical information. </li>
<li>Ability to effectively communicate with patient population and staff while demonstrating a high degree of diplomacy and tact.</li>
<li>Intermediate computing and phone skills.</li>
<li>Willingness to work past the end of shift to ensure patient care and safety; demonstrates flexibility in regards to job duties and assignments with a willingness to travel to necessary office locations.</li>
<li>Ability to multi-task and work effectively in a high-stress and fast-moving environment.</li>
<li>Culturally sensitive and demonstrated ability and effectiveness working with ethnically diverse populations.</li>
<li>Working knowledge of "Universal Precautions," demonstrates professionalism at all times.</li>
<li>Possess a thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA.</li>
<li>Bilingual English/Spanish and/or other languages strongly preferred.</li>
<li>Valid Driver's License, insurance, and ability to travel as required to perform duties.</li>
</ul>
EDUCATION AND EXPERIENCE:
<ul>
<li>High school Graduate (or GED) required.</li>
<li>Current, valid Certification of Medical Assistant through a nationally recognized accrediting agency such as the AAMA or RMA.</li>
<li>Current, valid CPR certification.</li>
<li>Familiarity with community / service area.</li>
<li>Previous experience as a Medical Assistant.</li>
<li>Electronic medical records experience preferred.</li>
