- MGB NWH Self-Study Orientation & Attestation required for onboarding
- Fit Test (N95) within 365 days of start date and updated annually
- Documentation of MMR vaccination or immunity required
- Influenza vaccine required during flu season (Sept 15 - April 30) with no exemptions
- TB test documentation within 3 months of start date and updated if >6 month break
- Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break
- MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break
- MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break
- MGB Resume including past 7 years of work experience required for submission
- Respirator Medical Clearance Attestation required if fit test is required
- MGB References: 2 recent references required for submission
- MGB Skills Checklist required for submission
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions
- MGB Education Verification through background check company
- MGB SSN Trace with criminal check
- MGB National Sex Offender Check within 30 days of start and updated annually
- MGB Exclusion Checks within 30 days of start and annually
- MGB TB Symptom Survey if positive PPD (optional)
- MGB Candidate Submission Coversheet required for submission
