LICENSED PRACTICAL NURSE POSITION SUMMARY
 The Licensed Practical Nurse (LPN) delivers quality care that is consistent within the scope of practice as outlined by the local state nurse practice act for Licensed Practical/Vocational Nurses. The LPN is responsible for tasks, activities and functions as delegated and may make assignment of duties to others as defined in their state of practice act. LICENSED PRACTICAL NURSE ESSENTIAL FUNCTIONS

The Case Manager provides case management services in a structured residential Substance Use Disorder setting, through education and life skills to help people transition in to sustainable recovery. This position is full-time: 36 hours per week, Monday, Wednesday, Thursday, Friday - evening shifts may be needed. 
What you’ll do:

<li>Supervise and/or participate with residents, based on their needs in self-help skills, activities of daily living, socialization, leisure time and community integration.</li>
