Primary Responsibilities:

<li>Visit shelters on a weekly basis to assist with housing applications and to ensure a seamless transition into CSP-HI program housing. </li>

We are seeking a Youth Family Foundations Case Manager to join CTI’s Youth Services team, located at the Youth Opportunity Center. The Youth Family Foundations Case Manager will provide advanced levels of support to young adults (14-25) and their growing families entering Youth Family Foundations programming. This includes providing advanced level case management, advocacy, referral information and supporting participants in overcoming barriers towards long-term, independent housing.
Responsibilities Include:

<li>Assess for general need for each family or individual</li>
<li>Connect to community supports geared towards family development</li>
<li>Increase access to nutritional supports</li>
<li>Connection to prenatal supports and in-home supportive services</li>
<li>Increasing access to education and employment</li>
<li>Creation of individualized goals with a youth-focused lens</li>
<li>Reduce barriers and overcome obstacles towards stability</li>
<li>Data tracking, entry, and submission into local and state systems</li>
<li>Support existing Youth Service programming and team members as needed</li>
</ul>
Qualifications:
<ul>
<li>Bachelor’s Degree preferred, preferably in a Human Services related field</li>
<li>2 years of experience working with youth and young adults preferred</li>
<li>Ability to work comfortably with pregnant and parenting youth in both group and individual settings, preferably with experience advocating for families</li>
<li>One-year assessment, advocacy and case management work in a Human Service field preferred</li>
<li>Demonstrated knowledge of community resources available to low-income individuals and families</li>
<li>Excellent customer service, organizational, oral and written communication skills</li>
<li>Ability to manage multiple priorities and work in a fast-paced environment</li>
<li>Proficient computer skills required in basic computer software and data collection systems</li>
<li>Excellent crisis management skills</li>

Job Title: In Home Behavior Monitor 
Work Location: New Bedford, Ma.
The Bachelor’s Level (Behavior Monitor) will work with youth under 21 years of age and their families in the home. They will work closely with an In-Home Behavior Therapist (IHBT) to treat challenging behaviors that interfere with the youth’s successful functioning. The Behavior Monitor will implement the behavior treatment plan, monitor the youth’s behavior, reinforce the implementation of the behavior plan by the parent(s)/guardian(s)/caregiver(s), and report to the IHBT on implementation of the behavior plan and progress toward behavioral objectives or performance goals
Duties and Responsibilities:
