The Residential Support Specialist participates in the engagement, assessment and treatment planning process and provides interventions to address needs and goals of persons served. The Residential Support Specialist provides direct services to people in a respite setting in the areas of daily living skills, transportation, skill-building, money management, medication administration, risk mitigation and other areas based on the person centered treatment plan. Granite State Independent Living (GSIL) has immediate per diem position openings for Attendant Care Employees (ACE) in Contoocook NH!
As an ACE for Granite State Independent Living, you can make a meaningful impact by providing compassionate support to our consumers and promoting their independence.
Flexible schedules: 20 hours a week (Schedules set by consumers).
Job Types: Per Diem.
Salary: $18.00 per hour.
ACE Qualifications include:

<li>LNA License is NOT required for this position. </li>
<li>High school diploma not required for this position. </li>
<li>Must be 18 years old.</li>
<li>Must be able to frequently lift, push and pull up to 50 pounds or more.</li>

ACE Duties and Responsibilities:
<ul>
<li>Activities of daily living (bathing, dressing, toileting, transferring, eating).</li>
<li>Provide in-home care. </li>
<li>Range of motion/exercises.</li>
<li>Laundry, meal preparation, housekeeping, organization etc. </li>
<li>Assisting with ambulation and transfers.</li>
<li>Use of mechanical lifting devices (will provide training if necessary).</li>
<li>Medication assistance (reminding a consumer of when it is time to take their medication).</li>
<li>Providing support in the community with running errands.</li>

Community Bridges is looking for a full-time and part-time Direct Support Professional in Franklin, NH! 
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.
In the Behavioral Day Program, our Direct Support Professionals support those who experience developmental disability and occasional behavioral challenges in their home and community. Each person regular daily routines and activities they enjoy in their community and they love to learn and see new things. The Direct Support Professionals provides support to the individuals by attending medical appointments, monitoring their health, safety, nutrition, grocery shopping, community activities, and community connections.
Qualifications:
<ul>
<li>Education: High School Diploma or GED required.</li>
<li>Experience: Experience in the human service or related field with persons experiencing disabilities desirable.</li>
</ul>
Reliable transportation, a valid driver's license, and proof of automobile insurance.
Compensation and Benefits:
There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role.
*Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Specific Duties and Responsibilities:
<ul>
<li>Assist individuals with planned activities and community involvement.</li>
<li>Develop a relationship map to gather information about the individual in order to establish social inclusion opportunities.</li>
<li>Assist individuals in developing a network of people in their lives. Maximize opportunities for the individuals they support with disabilities to get to know others and maximize the opportunities for others to get to know the individual.</li>
<li>Assist the individual in obtaining a sense of self-esteem through active participation in the community.</li>
<li>Provide individualized type and level of support to include, but not be limited to, personal care and/or skill acquisition etc.</li>
<li>Assist individuals in establishing, maintaining, and presenting a positive image. This may involve such activities as assisting in the bathroom, bathing, showering, and dental hygiene, etc.</li>
<li>Maintain a calm demeanor and respond quickly and appropriately to aggressive or verbal outbursts. Scan room and individual for potential danger.</li>
<li>Assist in developing and individualizing the yearly Person Centered Planning by providing information for assessments and by developing implementation strategies for goals/objectives and methods for monitoring the delivery and quality of supports to the individual.</li>
<li>Complete documentation requirements as specified by the Individual Service Plan to include the scheduled data collection and monthly progress notes to show an increase or decrease in behaviors to continually support individuals as they grow and reach goals.</li>
<li>Assist in the orientation/training of new staff, which may include introduction to an individual's support persons and to an individual's ISP, strategies, dreams and goals, and treatment plan.</li>
<li>Distribute medication in accordance with He-m1201 as applicable.</li>
<li>Report any safety, health or human rights concern immediately to the Supervisor.</li>
<li>Understand and ensure compliance with applicable state regulations.</li>
<li>Perform other duties and responsibilities as assigned.</li>
</ul>

Work Environment and Physical Demands:
<ul>
<li>Work environment- The positions is typically indoors, unless the individual wants to go outside. The noise level is minimal at the residence.</li>
<li>Physical demands- Direct Support Providers must be physically able to lift up to 50 lbs. and to provide transfers as necessary. Walking and standing required frequently with occasional stair climbing. Must be able to push wheelchairs and operate wheelchair lifts/Hoyer and barrier free lifts. Emergency behavioral intervention must be followed in accordance with behavioral intervention techniques and individual behavioral plans.</li>

Travel: 
Must have access to personal vehicle during all scheduled work hours. Frequently travels with supported individuals utilizing personal vehicle as well Community Choices vehicles. Must possess and maintain a valid NH license and safe driver's record in accordance with Community Choices standards with adequate liability insurance.
