Job Description: 
The Nurse Practitioner provides comprehensive, cost effective health care to adults which includes assessment, diagnosis, and treatment according to the standards of advanced nursing practice.  
 
Educational Requirements:

Master’s Degree in Nursing required.
Experience Required:

Speech Language Pathologists (SLP's) assess, diagnose, evaluate, and treat children with a wide variety of confirmed or suspected developmental disabilities. The Brockton Early Childhood Intervention Program is a public health program that provides therapeutic support to infants and toddlers who have developmental delays or are at risk for such delays. The goal of the program is to help children as soon as possible so they can reach their full potential. Services include speech therapy, occupational therapy, physical therapy, and skill development. 

Must be a licensed SLP in the state of MA , have a Master's degree, and a valid driver's license in state of residence with own means of transportation. Home visiting to surrounding towns is required.

RESPONSIBILITIES:
<ol>
<li>Evaluate the physical therapy needs of person served in a site-based day habilitation program</li>
<li>Perform physical therapy assessments: gait assessments, range of motion, strength and functional mobility</li>
<li>Complete physical therapy service needs assessment for each person served every 2 years or if there is a significant change in status</li>
<li>Make recommendations for suggested equipment designed to improve person served mobility</li>
<li>Develop exercise programs and activities designed to improve person served overall functional mobility</li>
<li>Provide training to staff and person served regarding the safe and effective use of adaptive equipment</li>
<li>Participate in the development of the Day Habilitation Service Plan and Semi-Annual Reviews as a member of the Interdisciplinary Team</li>
<li>Participate in Interdisciplinary Team meetings when appropriate</li>
<li>Complete Provider Support Strategies when necessary for any objective recommendations</li>
<li>Provide training to staff and person served regarding any physical therapy recommended objectives, if necessary</li>
<li>Maintain collaborative working relationships with person served support network</li>
<li>Attend meetings and appropriate trainings</li>
<li>Perform other related duties as needed or assigned by supervisor</li>
<li>Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk</li>

QUALIFICATIONS:
<ul>
<li>Massachusetts license as Physical Therapist required </li>
<li>Must possess a valid drivers license in the state of residence and have reliable transportation.</li>
<li>Ability to lift, transfer, push/pull, maneuver and reposition 25 pounds on a regular basis.</li>
<li>Must be able to stoop, squat, sit or otherwise position self to closely interact with persons served </li>
<li>Must be able to bend, reach, and stretch</li>
