POSITION SUMMARY:
Provides health care to the geriatric population in a specialized program and performs selective medical services within the Skilled Nursing Facility (SNF) setting through a collaborative agreement with a supervising MD/DO. Responsibilities include conducting annual wellness visits, along with other routine and specialized assessments, to ensure comprehensive care for older adults.
ESSENTIAL FUNCTIONS:
• Obtains complete medical history and physical examination on patients.
• Reviews medical records.
• Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.
• Establishes presumptive diagnosis.
• Orders appropriate laboratory and diagnostic procedures.
• Develops a therapeutic plan for patient.
• Sutures wounds.
• Prepares written prescription orders for drugs and controlled substances.
• Instructs patients and family regarding medications and treatment instructions.
• Provides recommendations regarding health and illness prevention.
• Recommend community resource to meet patient/family needs.
• Performs independent evaluations and treatment procedures in emergency situations.
• Provides monitoring and continuity of care between physician visits.
Maintains patient confidentiality
KNOWLEDGE, EDUCATION AND SKILL REQUIREMENTS:
- 3-5 years Primary Care Practice Experience or geriatric setting preferred
- Graduate of an accredited Acute Care Nurse Practitioner program.
- Licensed for Nurse Practitioner license in the state of Vermont.
- Strong inpatient skill set.
- Quality-focused team player.
- Excellent organizational, time management and problem-solving skills.
- Ability to function effectively as part of a multi-disciplinary team.
- Effective oral and written communication skills.
- Commitment to providing exceptional patient care.
