Position Overview:
The Case Counselor (ATS) will provide case management services and work with the clinical staff to coordinate care, educate on the continuum of care, and assist with treatment plans for clients.
Specific Responsibilities:

<li>Participate in the assessments and developments of the service plan.</li>
<li>Implements services to patients in accordance with their individualized service plan (ISP).</li>
<li>Collaborates with the patient and patient’s family or significant others to implement an effective service plan, explaining the available continuum of care and clinical options to the team, including the advantages and disadvantages of each option.</li>
<li>Complete clinical intake documentation as assigned.</li>
<li>Complete clinical assessment for risk per policy.</li>
<li>Communicates and documents patient’s progress toward their recovery.</li>
<li>Maintains the patient’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning).</li>
<li>Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patient, their family, behavioral and general medical and dental health care providers, community resources and others to ensure that all services prescribed in the individualized service plan (ISP) are implemented.</li>
<li>Provides assistance in maintaining, monitoring and modifying covered behavioral health services.</li>
<li>Works with all collateral contacts, reduce barriers to continuum of care engagement and meeting goals for ISP.</li>
<li>Ensures all appropriate referrals for identified services on the service plan are made and coordinated with service providers via collateral providers or community resources.</li>
<li>Participates in meetings regarding the continuum of care and discharge planning.</li>
<li>Work towards established goals and OKR’s for the case management department.</li>

Education and Experience:
High School Diploma and minimum of one (1) year of professional experience in Behavioral Health or Psychiatric is required Bachelor’s Degree in a social work field is preferred
Licensure is not required but a Licensed Drug and Alcohol Counselor (LADC) II or I strongly preferred
Knowledge of healthcare, detoxification process, addiction, co-occurring disorders, DSM Criteria, and terminology. A working knowledge of Microsoft Word, Excel, and Outlook required
Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.

This position is based in our Worcester office.&nbsp; &nbsp;The role offers a hybrid schedule of 4 in-office days and 1 remote day per week and is a full-time, 40 hour per week position.
Essential Duties and Responsibilities include:
<ul>
<li>Drafting of pleadings, motions, subpoenas, and correspondence</li>
<li>Managing service of process</li>
<li>Managing of documents on internal systems</li>
<li>Working directly with clients to compile documents and draft financial statements and discovery responses</li>
<li>Preparing alimony calculations and child support guidelines worksheets</li>
<li>Preparing asset division spreadsheets</li>
<li>Managing all aspects of discovery</li>
<li>Assisting in all aspects of trial preparation, including preparing trial binders</li>
<li>Meeting, assisting, and corresponding with clients</li>
<li>Conducting research on factual topics related to cases</li>
<li>Working directly with attorneys in docketing court dates and deadlines</li>
<li>Communicating with subpoena recipients, experts, GALs, ARC counsel, opposing counsel and parties, DCF, and the Courts</li>
<li>Other related duties as required</li>
</ul>
Qualifications and Requirements:
<ul>
<li>Minimum of five (5) years of Paralegal experience in a domestic relations and/or probate litigation practice</li>
<li>Preference for an individual with domestic relations and/or probate litigation experience in both Massachusetts and Rhode Island jurisdictions</li>
<li>Excellent organizational, communication, and interpersonal skills</li>
<li>Attention to detail and strong proof-reading skills</li>
<li>Time management and multitasking abilities</li>
<li>Strong research and writing abilities</li>
<li>Ability to use Microsoft Office and/or learn new computer programs and Apps for virtual meetings</li>
<li>A professional attitude and approach to client service are essential</li>
<li>Ability to work in a fast-paced environment</li>
<li>Must be able to perform duties with minimal supervision and also work as part of a larger team</li>
</ul>
Education: High School Diploma or Equivalent required
Associate Degree or higher preferred
Paralegal Certification preferred
