Lamoille Health Partners, a Federally Qualified Health Center (FQHC) committed to providing comprehensive, patient-centered care to underserved populations, is seeking a dedicated and compassionate Family Medicine Physician (MD/DO), Nurse Practitioner (NP), or Physician Assistant (PA) to join our team.

Responsibilities:

  • Provide comprehensive primary care services to patients of all ages, including preventive care, acute care, and chronic disease management.
  • Conduct thorough patient assessments, develop individualized treatment plans, and provide patient education.
  • Order and interpret diagnostic tests, prescribe medications, and perform minor procedures.
  • Collaborate effectively with other members of the healthcare team, including physicians, nurses, medical assistants, and behavioral health providers.
  • Maintain accurate and timely patient records in accordance with HIPAA and other regulatory requirements.
  • Participate in quality improvement initiatives and contribute to a culture of continuous learning.

Qualifications:

Required:

  • MD/DO, NP, or PA degree from an accredited program.
  • Current and unrestricted license to practice in Vermont.
  • 5 years or more of experience working in a health center or hospital setting.
  • Board certification in Family Medicine (MD/DO) or national certification as an NP or PA.
  • Current BLS/ACLS certification.
  • Excellent communication and interpersonal skills.
  • Strong clinical and problem-solving skills.
  • Commitment to serving underserved populations.
  • Ability to work effectively in a team-based environment.

Preferred:

  • Experience working in an FQHC or community health center setting.
  • Experience with electronic health records (EHR) systems, specially eClinicalWorks.
