Job DescriptionBereavement Coordinator (Counselor)
Acclaim Home Health is seeking a compassionate and organized Bereavement Coordinator (Counselor) to support hospice patients’ families and caregivers through the grieving process. This role is responsible for coordinating and delivering bereavement services before and after a patient’s passing, ensuring families receive emotional support, education, and appropriate resources.
The Bereavement Coordinator plays a key role on the interdisciplinary hospice team, helping families navigate loss with dignity, care, and compassion.
Responsibilities
- Provide grief counseling, emotional support, and education to hospice patients’ families and caregivers
- Develop, coordinate, and manage bereavement care plans in accordance with hospice standards
- Conduct bereavement risk assessments and ongoing follow-up with families for up to 13 months post-death
- Facilitate or coordinate support groups, memorial services, and bereavement outreach activities
- Collaborate with the interdisciplinary hospice team to ensure continuity of care
- Provide crisis intervention and referrals to community grief resources as needed
- Maintain accurate and timely documentation in compliance with hospice regulations and company policies
- Ensure compliance with state, federal, and hospice regulatory requirements
Qualifications
- Bachelor’s degree in Counseling, Social Work, Psychology, or a related field required
- Master’s degree preferred (Counseling, Social Work, or related discipline)
- Experience in bereavement counseling, hospice, mental health, or human services preferred
- Strong interpersonal, communication, and organizational skills
- Ability to work independently and as part of a multidisciplinary care team
- Compassionate, empathetic approach to grief and loss support
- Reliable transportation and ability to travel locally as needed
