Lynn Community Healthcare Center is seeking a dynamic Population Health Director to reimagining access to high-quality care and improved outcomes across channels. Key Responsibilities:
- Human-Centered Approach: Focus on creating impactful and meaningful work that prioritizes the needs and experiences of patients.
- Access to Care: Develop and implement strategies to enhance access to high-quality care across the continuum.
- Data Analysis: Leverage and translate complex data sets into actionable insights driving priorities for improvement and health outcomes.
- Critical Thinking: Assess, identify, and define barriers to care and opportunities to address social determinants of health.
- Innovation: Create and implement innovative programs to improve access to care and outcomes.
Basic Qualifications
- Bachelor’s Degree in Healthcare Administration, Business Administration, Public Health, or a related field, or equivalent experience.
- 5+ years of experience in healthcare operations, panel management, VBC or related field, with a strong emphasis on population health, or related areas.
- 2+ years of experience in leadership, overseeing teams and projects.
- Demonstrated experience in implementing operational improvements and achieving measurable outcomes in healthcare settings.
- Strong understanding of healthcare operations, panel management strategies, and caregiver / consumer integration.
- Strong analytical skills with the ability to interpret data and trends, diagnose issues, and formulate actionable recommendations.
- Effective communication skills, both verbal and written, with the ability to convey complex information clearly and persuasively to diverse audiences.
Desired Qualifications
- Master’s Degree in Business Administration or a related field.
- Proven track record of designing and implementing sustainable systems to improve access, efficiency and optimize healthcare operations and outcomes.
- Demonstrated ability to effectively communicate complex concepts and system designs to diverse stakeholders within the organization, including executives, clinical staff, and administrative teams.
- Experience in leading change management initiatives to foster adoption and integration of new systems and processes across multiple departments or facilities.
- Track record of developing comprehensive programs and resources to educate and empower staff at all levels on system functionalities and best practices.
