Job Description:
POSITION EXPECTATIONS (Includes but is not limited to):Evaluates, assesses, and plans nutrition care to meet patient nutritional needs.
- Assesses patient nutritional status using documented medical information and established nutritional standards.
- Counsels patients and/or families by identifying nutrition-related problems. Develops plans of instruction and nutritional care, and monitoring results.
- Identifies inpatients at nutritional risk and documents recommendations.
- Diagnose and develop intervention for patients with malnutrition
- Participates in physicians' rounds, discharge planning rounds, and conferences with nutritional implications.
- Conducts education programs for patients, community groups, and staff.
- Precept and mentor new staff or existing staff in clinical specialty area.
- Develop therapeutic relationships with multiple subspecialty care team members and providers that enables planning and participating across a broader spectrum patient care.
- Cross-covers a broader spectrum of patient care areas and subspecialties as needed.
- Performs other duties as required or assigned.
Submission Requirements:
3+ years CURRENT Hospital Dietitian experience
- Rural hospital dietitian experience required
- Current VT Dietitian license
- Current Registration by the Commission on Dietetic Registration
- Bachelor degree in Nutrition from accredited college/university required
