Post Acute Care Physician / Primary Care Physician - Springfield, MA

Job Description

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.

Offerings include care for a range of patients from short-term rehab to complex geriatrics, aiming to improve access, remove barriers, and achieve optimal clinical outcomes.

As part of the team, you will deliver primary care, participate in educational sessions, and collaborate with nurse practitioners and physician assistants.
