In this role, you’ll be responsible for delivering comprehensive inpatient and outpatient-quality support in a long-term care setting. Your day-to-day duties may include conducting resident assessments, administering medications and treatments under supervision, monitoring vital signs, and coordinating with RNs, physicians, and interdisciplinary team members to adjust care plans as conditions change. You’ll support daily living activities, assist with mobility and rehab exercises, manage wound care and symptom relief, and maintain meticulous documentation to ensure accuracy and continuity of care. You’ll emphasize safety, fall prevention, infection control, and resident dignity, consistently communicating with families and teammates to align on goals.
