Overview
The Resource Navigator is an administrative position that assists patients and families with navigating and accessing external resources, support systems, and information by telephone or in person. The Navigator is responsible for providing clerical support, assisting the team with problem-solving and facilitating inpatient and outpatient care. This position proactively addresses potential or actual transportation and lodging needs as identified by the interdisciplinary team. The Resource Navigator identifies the need and coordinates the assistance of a bilingual interpreter, and assists international patients by facilitating Visas, airline letters and supporting documentation.
The pay range for this position is $20.56/hourly to $27.96. Compensation is determined based on years of experience and department equity.
Responsibilities
- Works to support the Care Management team, by assisting patients and families with transportation and lodging needs, navigating and accessing internal and external resources, support and information
- Supports communication with Medical Homes as needed
- Works with the team to ensure documentation of assessments, interventions, plans and education in the patient's Electronic Medical Record
- Participates as part of a multidisciplinary team to plan future episodes of care for patients
- Identifies need for and coordinates assistance of outside interpreters
- Assists the Care Management team, family and patients to optimize utilization of insurance benefits
- Supports the coordination of durable medical equipment
- Coordinates the needs of international patients visits/treatment by facilitating Visas, airline letters, and supporting documentation

Qualifications
Required
High School diploma
Preferred
Bilingual English/Spanish
Customer support in a pediatric medical setting
Medical Terminology experience
