Care Coordinator – CCBHC
LUK Care Coordinators provide strength-based, client centered, trauma informed care coordination services to support and build capacity of Certified Community Behavioral Health Clinic (CCBHC) consumers and their families. This position meets with consumers and families on a regular basis to ensure adherence to the individual treatment plan and to assess progress in identified goals. The Care Coordinator works closely with internal and external team members to achieve positive outcomes This position supports the development of assessments, engages in consultation with collateral sources, and connects consumers to community resources to achieve short- and long-term client goals. The CCBHC Care Coordinator will be expected to participate in training for EBPs as needed for this position, and ensure fidelity to the identified models. This is a full-time position with some non-traditional hours expected.

Requirements:
Purpose:
LUK’s CCBHC Care Coordinator will be part of an innovative team seeking to ensure equitable, quality, person centered and strength-based service provision to consumers with mental health and/or substance use needs.
Responsibilities Include
- Meet with consumers/families in the office or community regularly
- Maintain communication with consumers/families between meetings
- Provide care coordination by collaborating with internal and external members of a multidisciplinary team to support consumers and families in achieving their goals
- Complete or assist with intakes and discharges
- Complete evaluation documentation as required, including the NOMS
- Identify consumer needs and connect them with community resources to meet those needs
- Complete regular reviews of progress toward goals to quantify consumer/family functioning
- Complete evidence-based practices training(s) and maintain fidelity to identified model(s)
- Participate in consumer- and family-centered treatment team meetings and other necessary meetings
- Provide trauma-informed services (individual, family, crisis)
- Model skills for consumers/families and coach/mentor them to develop their own skills
- Actively participate in supervision and training as required
- Assist with gathering information for internal reports for the program funder as requested
- Participate in the emergency response team as scheduled
- Gain an understanding and commitment to LUK’s Mission, Vision, and Values and provide congruent clinical services
Job Location:
The Care Coordinator is based in LUK’s main office in Fitchburg. This is an office based, in-person position, with some flexibility for occasional hybrid work. The Care Coordinator is expected to be available to travel for home or community-based appointments within the 27 cities and towns of North Central Massachusetts.
Salary Range and Benefits:
Pay Range: 9B. Salary Range: $49,000 – $57,000. Salary ranges within each pay grade are based on qualifications, skills, and experience.

Knowledge and Experience Required:
- Bachelor’s Degree in a human services field, lived experience may be substituted
- Experience in delivering services to individuals with mental health and substance use challenges
- Experience implementing evidence-based practices
- Experience working with system-involved families
