HOME HEALTH PHYSICAL THERAPIST
Job Title: Physical Therapist – Home Health
Rate: Up to $2300.00 weekly
Location: Agawam, MA
Day (8H) - (5x8hr) - (8A - 5P)
Key Responsibilities
<ul>
<li>Patient Assessment: Evaluate patients' physical function and limitations within their home environment to develop individualized treatment plans.</li>
<li>Treatment Implementation: Provide therapeutic interventions aimed at improving mobility, strength, balance, and pain management.</li>
<li>Education: Educate patients and caregivers on diagnoses, safety precautions, equipment use, and home exercise programs.</li>
<li>Collaboration: Work with an interdisciplinary team, including nurses and occupational therapists, to create comprehensive care plans that address the patient's physical, emotional, and social needs.</li>
<li>Documentation: Maintain accurate records of patient evaluations, treatment plans, and progress according to healthcare standards. </li>
</ul>
Education
<ul>
<li>Required a degree in physical therapy (Bachelor's or Master's)</li>
</ul>
Experience
<ul>
<li>Previous experience in home health care of similar setting is preferred.</li>
</ul>
Licenses/Certifications
<ul>
<li>Must hold a valid state license to practice physical therapy</li>
<li>Must be able to obtain and maintain CPR certification.</li>
</ul>
