• Coordinate care with Family Partners, Paraprofessionals, schools, medical providers, and community agencies.
• Maintain timely, accurate documentation compliant with agency, CBHI, and MassHealth standards.
• Participate in interdisciplinary team meetings, supervision, and required trainings to ensure fidelity to the FIT model.
• Communicate proactively with families; offer advocacy and system-navigation support.

• Demonstrated trauma-informed and culturally responsive practice.
• Strong clinical assessment, diagnostic, and treatment-planning skills; crisis intervention and family engagement experience.
• Proficiency with electronic health records and Microsoft Office.
• Current CANS (or ability to obtain prior to start, per program policy).
• Valid driver’s license, insured reliable vehicle, and ability to travel locally; ability to work some evenings/weekends and participate in on-call rotation.

Nice-to-Haves
• Familiarity with CBHI services and FIT/wraparound models.
• Bilingual/bicultural candidates strongly encouraged to apply.
