Consolidated Medical Travel is seeking a travel Physical Therapist for a travel job in Pittsfield, Massachusetts.

Important RequirementsCOVID-19 Vaccination

  • COVID vaccination (dose dated on or after 10/3/23) is required.
  • Exemption forms (medical or religious; both require provider/religious leader signatures) are available in Trio → Downloadable Forms or from your delivery team.
  • Approved exemptions require masking at all times on-site.
  • To avoid processing delays, submit vaccination card or intent to complete exemption with your profile.

Traveler Radius Restriction

  • Candidates living within 50 miles of the facility will NOT be considered.
  • Include permanent address at the time of profile upload.

Requested Time Off (RTO)

  • All RTO must be submitted at profile upload.
  • Post-upload RTO requests will NOT be approved.
  • Maximum of 5 days RTO allowed (fewer preferred).

Holiday Coverage Requirement

Travelers must work TWO of the following holiday blocks:

  1. Thanksgiving + Friday/weekend after
  2. Christmas Eve/Christmas Day + Friday/weekend after
  3. New Year’s Eve/New Year’s Day + Friday/weekend after

Note: Assignments ending in November or before the December holiday period will not be considered for extension into 2026.

Interview Requirements

  • At least TWO specific interview dates/times must be entered in Trio/profile.
  • “Anytime” will NOT be accepted.

  • The Physical Therapist is responsible for providing direct physical therapy services to home health clients. Primary duties include:
  • Delivering in-home patient care and collaborating with the interdisciplinary care team.
  • Participating in development of the patient’s comprehensive plan of care.
  • Managing and training patients on rehabilitation equipment, including:
    • Splints
    • Orthoses
    • Prostheses
    • Wheelchairs
    • ADL equipment
    • Modalities
  • Working independently with minimal supervision.
  • Ensuring accurate and timely documentation in accordance with home health standards.
  • Candidate must be proficient with OASIS (non-negotiable requirement).
