Key Responsibilities:

Evaluate residents and develop individualized treatment plans

Provide therapeutic interventions to improve mobility, strength, and overall function

Collaborate with an interdisciplinary care team

Maintain accurate and timely documentation

Educate residents and families on therapy goals and home programs
Qualifications:

Active MA PT license

SNF experience preferred but not required
