ABOUT THE POSITION
The Stabilization Case Manager supports families as they transition from shelter to permanent housing and continues services for up to two years to prevent homelessness and promote long-term stability. Using a strengths-based, person-centered approach, the Case Manager meets families in shelter and at home, builds individualized service plans, connects families to resources, and collaborates closely with landlords, courts, health providers, workforce partners, and CSR teams to improve family well-being and housing stability.

KEY RESPONSIBILITIES
• Conduct monthly home visits across Boston; make referrals to community resources and partner toward long-term stability and family independence.
• Complete CSR stabilization assessments; establish and monitor EOHLC service plans that address barriers to maintaining housing.
• Coach families on lease compliance and stabilization plans; mediate with landlords; provide housing counseling and referrals to prevent loss of housing.
• Coordinate legal services and support appeals/mediation; attend housing court as needed for subsidy termination or eviction matters.
• Increase economic self-sufficiency by partnering with CSR Workforce Development, supporting financial literacy, budgeting, savings, and benefits enrollment.
• Support access to health coverage and primary care; make referrals and manage crises per agency policy.
• Maintain complete, timely records and required data reporting (e.g., HUD assessments—intake/update/exit, case notes, Stabilization Housing Plan, incident reports).
• Provide quarterly collateral check-ins; maintain positive relationships with landlords and housing providers.
• Perform other duties as assigned.

REQUIREMENTS
Must-Haves
• Written and verbal fluency in Haitian Creole.
• Reliable transportation for home visits and meetings; willingness to travel locally, including meeting families in their homes, which may require climbing stairs
• Experience working with formerly homeless or low-income families; knowledge of Boston-area community resources.
• Demonstrated ability to set boundaries, maintain confidentiality, and work respectfully with families experiencing homelessness.
• Strong interpersonal skills (active listening, empathy, calm in crisis); effective oral and written communication.
• Ability to work independently, use supervision/consultation in decisions, and adapt to changing circumstances.

Nice-to-Haves
• Bachelor’s degree in Human Services or related field.
• Data entry experience in ETO or Apricot (or similar systems).
