Healthcare for the Homeless is looking for a compassionate and self-motivated individual to join our team as a Behavioral Health Coordinator (BH) Coordinator. The BH Coordinator will provide guidance, oversight and supervision for all Behavioral Health Providers at the Health Care for the Homeless (HCH) Program of Manchester, as they engage in direct patient care and collaborate with other members of the HCH team to address mental health, substance use disorder and concrete service needs of patients. The BH Coordinator will provide guidance, for Street Outreach Team, Medication Assisted Treatment program, Outreach/Enrollment efforts, and Health Education endeavors to ensure effective operations and adherence to program requirements.
Responsibilities:
Supervision & Quality Management
· Supervise Behavioral Health (BH) providers, offering daily support, regular supervision, and conducting annual performance reviews.
· Help eligible staff pursue licensure (LICSW, LCSW, LCMHC, MLADC) in New Hampshire.
· Work with leadership (Medical Director, Executive Director, Practice Manager) to evaluate the HCH program, address issues, improve systems, and develop policies for quality care and program performance.
· Integrate BH care with clinical, outreach, and education efforts, and guide team discussions during case management meetings.
· Participate in the Quality Improvement (QI) committee to review outcomes and implement needed changes.
Behavioral Health Counseling
· Conduct BH screenings and assessments with the medical team; respond to referrals and provide immediate support during clinic sessions.
· Identify needs related to mental health, substance use, and social support through screenings and psychosocial assessments.
· Develop care plans using DSM-5 and ASAM guidelines; recommend appropriate levels of care.
· Collaborate with team members to implement and monitor care plans.
· Offer counseling, support, crisis intervention, and education in individual, couple, and group settings.
· Coordinate referrals to mental health, substance use, and respite care providers.
· Record all assessments and treatments in the EMR, ensuring compliance with coding, quality goals, and data tracking standards.
Concrete Services
· Identify and respond to patients’ social and economic needs, including housing, benefits, and employment support.
· Help patients access services and follow up on referrals.
· Work with hospitals and ERs to coordinate discharge planning for homeless patients.
· Partner with community providers to support care access and patient enrollment in HCH.
Requirements:
· Master’s Degree in Behavioral Health from an accredited program required.
· Minimum of two (2) years of experience in a Behavioral Health or Outpatient Community Health Care setting.
· Licensed Clinical Mental Health Counselor (LCMHC), Licensed Independent Clinical Social Worker (LICSW)/Licensed Clinical Social Worker (LCSW) in the state of New Hampshire.
· Valid Driver’s License or the ability to transport self to and from off-site locations.
