Job Description\n
American Traveler is seeking a Home Health Physical Therapist with home health certification and proficiency in OASIS for patient care in the community.
\n \n Responsibilities\n
- Home health setting providing direct patient care in clients' homes
- Works with rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities
- Independent work with minimal supervision required
- Requires travel between patient homes
- Primary responsibilities include direct patient care, collaboration with team members, and participation in developing comprehensive plans of care
- Must be able to work independently and manage cases with minimal supervision
- Candidates who reside within 50 miles of the facility will not be considered
- A maximum of 5 days of requested time off will be considered

- Certified Home Health experience required
- Proficiency with OASIS documentation is required
- Minimum 2 years of acute nursing care (hospital base) and home care experience
- Current Massachusetts physical therapy license required if applicable
- Current BLS certification (AHA) required
- Valid driver’s license required
- Acute care experience preferred
