Job DescriptionDescription:

Intensive Care Coordination is based on the well-known principles of Wraparound Care. This is a program that respects and incorporates the needs and choices of the family and youth with a focus on strengths and family culture. The supporting Care Plan Team (CPT) is chosen by the family and the team helps to inform the family on available resources, supports, and interventions that can support the family goals.


Care Coordination has been shown to have more positive outcomes with improvement to the lives of children and families and is a key component of the Children’s Behavioral Health Initiative (CBHI) system. Care Coordinators work as a team with a Family Partner which is an individual with lived experience of parenting a child with emotional and/or behavioral needs.


Qualifications:

Master’s Degree in a mental health field (including, but not restricted to, counseling, family therapy, social work, psychology, etc.) from an accredited college or university.

Bachelor’s Degree in a human services field from an accredited college or university and one year of relevant experience working with families or youth.

Requirements:

Purpose:

Individuals that qualify for this position will provide Intensive Care Coordination to youth with a serious emotional disturbance and their families. The Intensive Care Coordinator (ICC) works in partnership with a Family Partner to help the family identify the needs and develop, coordinate, and implement services and supports for the enrolled youth and their family using the Wraparound service model planning process as defined by the Massachusetts Children’s Behavioral Initiative (CHBI).


Responsibilities Include:

  • Acquiring a deep understanding and commitment to LUK’s mission, Vision, and Values and programming and advocacy.
  • Completion of assessments including Comprehensive Assessment, MA CANS, Safety Planning, and Risk Assessment
  • Creation of Care Plan with input from youth and family, as well as other formal/natural support team members
  • Coordination of multiple services and supports for the children and youth under the age of 21 that also have emotional/behavioral needs to develop an integrated Care Plan with the team.
  • Facilitate Care Planning Team meetings and learning/implementing the Wraparound Model of Care.
  • Act as a liaison with partnering agencies, State agencies, school personnel, etc.
  • Meet with youth and the family in their home, at the office, or in the community.
  • Provide referrals and linkages to appropriate services that the family has identified as being interested in receiving.
  • Support Caregivers to learn effective advocacy skills.
  • Identify and help the family to engage with natural supports on the team.
  • Communication with MassHealth Insurance representatives to review the family needs and obtain ongoing insurance authorizations to continue working with the family.

Knowledge and Experience Required:

  • A minimum of 1 year of experience working with youth and families
  • A demonstrated passion and commitment for working with youth and families
  • Master’s/Bachelor’s degree in a human services related field
  • Coordination of multiple services/supports for children and youth under the age of 21 with emotional/behavioral needs to develop an integrated Care Plan with the youth and family
  • Facilitation of Care Plan Team meetings
  • Acting as a liaison with partnering services including but not limited to State agencies, schools, hospitals/CBAT, etc.
  • Meeting with youth and family in their home or other location of their choosing in the community
  • Supporting caregivers with education regarding effective advocacy and empowerment
  • Participation in On-Call Services team
  • Identifying and engaging with family with the natural support on the Care Plan Team

Skills and Abilities:

  • Strong communication skills
  • Internet access for remote work when necessary
  • Reliable and Safe Transportation
  • Demonstrated ability to create meaningful and appropriate relationship while maintaining professional boundaries
  • Strong presentation skills
  • Self-starter with great organizational skills and drive to succeed
  • Ability to work independently, with weekly supervision
  • A demonstrated commitment to empowerment of children, families, and adolescents to realize their fullest potential
  • Knowledge of state system is a plus
  • Knowledge of resources within the community to address social policies and concerns for clients

Salary Range and Benefits:
Pay Range: 9B. Salary Range: $49,000 – $57,000. Salary ranges within each pay grade are based on qualifications, skills, and experience.
