HonorVet Technologies is seeking a travel nurse RN Case Management for a travel nursing job in Bradford, Massachusetts.

Description:

Must have MA license in hand for acceptance
1-year plus experience in Case Management, LTAC/Rehab
• CM experience must be specific for hospital discharge planning (rehabilitation hospital experience a plus but not required)
• Familiar with EMR and/or documentation expectations for role (ACMA SOP)
• Familiar with Continuity of Care / Transition of Care requirements (CMS standards )

Shift: Mon-Friday 8-5p **Shift is 5x8 days but any variation to 8 hour shift (staying late/coming in early) has to be discussed and approved by manager
