Job Description

The Triage Clinician plays a key role in helping people requesting services navigate LUK’s behavioral health services system. The Triage Clinician is responsible for conducting initial screenings, clinical triage assessments, and care coordination for children, youth, and their parents/guardians as well as young adults and adults requesting behavioral health services from LUK. The Triage Clinician primarily provides services by phone. This position engages with persons requesting services and ensures timely access to appropriate levels of care, in alignment with medical necessity criteria, MassHealth requirements, and agency standards. This position is key in helping people access the right level of care at the right time, from traditional outpatient services to intensive home- and community-based services (e.g., CBHI) and other behavioral health services, and by facilitating a smooth transition into services and programs.

The Triage Clinician serves as the first clinical point of contact for individuals and families requesting behavioral health services from LUK. The Triage Clinician primarily provides services by phone. The role ensures timely and accurate assessment of strengths, needs, risks, and eligibility while connecting consumers to the most appropriate level of care, whether through intensive community-based services or standard outpatient therapy. By increasing timely access to services, providing clear guidance on service options, and facilitating smooth transitions into care, the Triage Clinician supports the agency’s mission to deliver trauma-informed, family-centered, and culturally responsive behavioral health services within a coordinated system of care.

Knowledge and Experience Required

· A minimum of 3-5 years of experience working with youth and families.

· A demonstrated passion and commitment for working with youth and families.

· Master’s degree in Social Work, Counseling, Psychology, or related behavioral health field.

· Independent or licensed-eligible in Massachusetts: LMHC, LICSW, LMFT or equivalent.

· Must be CANS-MA certified/eligible.

· Experience with child/adolescent mental health and family systems.

· Strong assessment, crisis management, and clinical documentation skills.

· Knowledge of community resources, and how to help consumers access them.
