Pay: $46.00-$51.00 per hour with a retention bonus available up to 10K for qualified staff.
Job description:
Responsibilities:
- Telephone triage
- PRN visits for symptom management needs and other visits assigned by the Clinical Manager
- After-hours admissions and screens
- Remote chart auditing
- Stand up and stand down participation
- Roughly 5-6 visits a day, less if an admission is completed.. Can work 4 - 10 hour days

Required qualifications:
- Registered Nurse licensure in the Commonwealth of Massachusetts in Good Standing
- Prior experience as a nurse working for a licensed hospice or one year of acute care or sub-acute care experience
- Willingness to travel in your own reliable vehicle (mileage reimbursed) within our service area to provide direct care for our patients
- Prior experience delivering nursing care in the home setting or have an earned BSN or higher degree
- Experience with Point of Care EMR documentation
- Current Certification in CPR /Basic Professional Life Support
Preferred qualifications (not required, and we are willing to provide training to the right candidate):
- Familiarity with WellSky Hospice Electronic Medical Record
- Experience with case management for a patient panel of 10-20 patients.
- Experience with supervising Certified Nursing Assistants, Home Health Aides, and Licensed Practical Nurses
- Certification as a Hospice & Palliative Care Nurse and/or as a Certified Case Manager
