VNA Care is hiring a Patient Access Team Lead for our Worcester office.
Position Summary: Provides oversight of staff and processes in support of the team when supervisors are not available. Provides exceptional customer service for all internal and external customers including referral sources such as case managers, physician offices, hospitals, skilled nursing facilities as well as patients and patient families. Responsible for gathering all needed data in order to accept a referral.
Pay Range: $26.00 - $30.00 per hour.
Schedule: 30 hours a week/ Every other weekend:
Week 1= Weds/Thurs/Sat = 10-hour days
Week 2 = Sun/Weds/Thurs = 10-hour days
Week 3 = Weds/Thurs/ Sat = 10-hour days
Week 4 = Sun/Weds/ Thurs = 10-hour days
Reports to: Admissions Coordinator Supervisor
Functions & Responsibilities:
- Provides direction to staff regarding processes and support for the referral process.
- Updates schedules as needed in the event of staffing changes, ie sick calls, activate on-call as needed.
- Demonstrates basic understanding of Home Health and Hospice eligibility criteria for purposes of referral coordination.
- Answers calls from liaisons and pod team staff effectively and in a timely manner upholding service excellence standards.
- Updates tables in HCHB as needed including address mapping, physician updates and internal agency transfers
- Coordinates referrals from various electronic referral systems.
- Provides excellent customer service when answering phone calls and processes referrals in a manner that meets the organization’s goal to exceed customer expectations
- Initiates referral documentation of a pre-determined set of data elements in the EMR from all referral sources and assigns to the appropriate next person in the referral management process.
- Ensures that consistent and timely initial communication regarding referrals occurs within the PAC and with all Agency offices or service lines.
- Communicates with patient’s physician, family and others to obtain complete referral information and to assist in organizing resources necessary for patient care prior to admission; including discharge paperwork
- Assists with resolving referral and admissions issues through direct communication with referral sources and clinical admissions coordinator.
- Completes a daily reconciliation of referrals to ensure all work is completed for timely patient admissions.
- Demonstrates knowledge of a consistent application at all times of patient rights to confidentiality.
- Demonstrated ability to comply with HIPAA regulations and maintain effective workflow systems.
- Demonstrates a high degree of commitment to customer service and quality care provision.
- Applies effective, respectful and positive verbal and written communication methods in the daily work setting to promote a collaborative work environment.
- Attends periodic meetings and/or trainings.
- Meets productivity expectations and standards.
- Demonstrates knowledge of the Agency’s telephone system.
- Identifies and escalates priority issues.
- Weekend/Holiday rotation coverage is required.
- Upholds the compliance objectives, policies and procedures of VNA Care and Subsidiaries.
- Performs other related duties of a similar nature and complexity as directed.
Education:
- High School Diploma or GED required; Associate’s Degree preferred.
- Medical Assistant or Medical Secretary Certificate preferred.
Experience:
· Minimum of 1 year working in a health care environment preferred.
· Strong customer service background via telephonic communications required.
· Strong clerical and computer skills required including data entry, working knowledge of office machines, phones and filing systems.
· Experience using Excel preferred.
· Must have excellent written and verbal communications skills and strong command of the English language.
