Position Overview
We are seeking an experienced Licensed Practical Nurse (LPN) to provide direct patient care in a Long-Term Care (LTC) setting in Newport, VT. This role involves working collaboratively with Registered Nurses, physicians, and interdisciplinary care teams to deliver safe, high-quality, resident-centered care in accordance with organizational policies, state regulations, and safety standards.
Key Responsibilities

Administer medications and prescribed treatments per physician orders

Monitor, assess, and document resident conditions and responses to care

Assist residents with activities of daily living (ADLs)

Collaborate with Registered Nurses, physicians, and interdisciplinary team members

Identify and promptly report changes in resident condition

Follow individualized care plans and established clinical protocols

Maintain compliance with state regulations, safety standards, and organizational policies
Experience Requirements
-
Minimum 2 years of Long-Term Care (LTC) and/or Rehabilitation experience required
Certifications & Licensure
-
Active Licensed Practical Nurse (LPN) license (state-specific, in good standing)

Current BLS/CPR certification
Required Skills
-
Strong clinical judgment, communication, and documentation skills

Ability to work full 12-hour day shifts and meet contract requirements
