Recovery Support Navigator (Temporary Position)

The Recovery Support Navigator (RSN) helps patients navigate systems, develops a service plan based on those identified needs, facilitates access to social service benefits and other internal and external resources, monitors the patient's progress, and advocates on behalf of the patient to ensure completeness and continuity of care. The RSN works collaboratively with team members and providers, reaches out to patients for scheduling and monitoring of continued progress, and tracks relevant data points as required. Responsible for documentation in the electronic medical record and complying with all data entry, data integrity, and data tracking requirements for LCHC and funding agencies' contract monitoring reports as required.


ESSENTIAL RESPONSIBILITIES / DUTIES:

Addressing Needs

  • Addresses patients’ needs and goals of funding agencies
  • Monitors patients' progress via patient's level of functioning, adherence to treatment plans, recovery/relapse process and/or service needs
  • Assists with scheduling outpatient visits and follow-up as needed
  • Provides referrals and supports clients using a variety of culturally, linguistically and educationally appropriate strategies, in a variety of settings. Coordinates with Interpreter Services, as needed
  • Develops and maintains strong relationships with the community and resources to ensure patient access
  • Collaborates and coordinates with health care team members, including social work, to promote positive prevention, harm reduction.

EDUCATION:
Bachelors or Associate's degree (or equivalent combination of formal education and experience in community health) plus a minimum of 1 - 2 years in healthcare is preferred.

EXPERIENCE:
Preference for 2+ years of experience working in a health care setting, preferably with individuals with substance use disorders.
