Role specifics and growth: Your responsibilities include comprehensive assessment, medication administration, wound care, ADL assistance, mobility support, and vigilant monitoring of residents’ conditions to identify changes early. You’ll participate actively in care planning, document thoroughly in electronic records, and coordinate with an interdisciplinary team to optimize mobility, nutrition, pain control, and safety. You’ll mentor CNAs and collaborate with therapists and social workers to align clinical goals with personal goals, enabling smoother transitions between care phases, admissions, and discharges.
