Essential Functions: To provide support in day programs and in the community that enable individuals served with developmental disabilities and/or brain injuries to develop and maintain skills that foster personal growth and independence. 
Responsibilities:

<li>Assist individuals served with personal care tasks, which include, but are not limited to: toileting, grooming, personal hygiene, assisting with mobility, repositioning, dressing, and range of motion exercises.</li>
<li> Support individuals in the completion of Activities of Daily Living (ADL) and Independent Living Development tasks, which include but are not limited to: cooking, cleaning, general maintenance, shopping, money management, safety and self- preservation.</li>
<li> Utilize Positive Behavior Supports (PBS) practices, to create a thoughtful and supportive environment by ensuring activities, plans and routines reflect the preferences, needs and goals of individuals.</li>
<li> Ensure individuals’ nutritional needs are met according to a prescribed diet. Assist with feeding as necessary.</li>
<li> Use supportive and adaptive, health-related protections/equipment as prescribed. Clean equipment as directed.</li>
<li> Learn and utilize communication tools which include but are not limited to email, tablets, letter boards, etc. taking into consideration the method most suited to individual needs.</li>

Direct Support Professional 2 (DSP 2)

Job Description
As a Residential Support Counselor, you will provide direct care to individuals living in a residential setting. The RSS will help individuals with daily living skills, transportation, skill-building, money management, medication administration, risk mitigation and other areas based on the person centered treatment plan. Services are provided based on Recovery tenets and are aimed to promote overall health and well-being of persons served. $2.00/hour differential for high intensity programs
What you’ll do:
<ul>
<li>Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management and develop and/or restore skills impacted by mental illness to help them live a self-directed life.</li>
<li>Assist people with psychiatric and substance use disorders to identify personal priorities, preferences, strengths, and interests in order to help them establish goals that support a life in recovery.</li>
<li>Arrange and transport residents to appointments or daily outings.</li>
<li>Complete administrative tasks, such as answering phones or filling out paperwork.</li>
<li>Assist with household tasks such as cooking and cleaning</li>
<li>Provide side-by-side support and coaching to help people socialize to socialize and become more involved in their community</li>

What you’ll need:

 North Suffolk is looking for Direct Support Professionals to join our team! Direct Support Professionals are members of a team responsible for providing high quality direct care services to adults with developmental disabilities. What you’ll do:

<li>Offer guidance for residents during skills building</li>
<li>Develop and implement individual treatment plans for residents</li>
<li>Assist with daily living activities, including meal preparation, money management, doctor’s appointments and assisting with personal hygiene</li>

What you’ll need:

<li>Experience:

<li>Zero to six years related experience (depending on DSP level) providing child care/elder care/direct support to developmentally disabled persons or previous experience supporting individuals in a mental health/special education or other human services setting is required.</li>

Job Summary
The Resident Care Director (RCD) is responsible for the overall safe operation of the resident services department. The Resident Care Director plans, implements and evaluates the safe operation of the resident care department, staff, and residents of the community. The RCD will continuously evaluate the health functioning of all residents, and design and revises the care plan for each resident as necessary. This position ensures compliance with all applicable regulations and company policies and procedures. 
 
Duties and Responsibilities
The following essential functions are the fundamental job duties of the position to be completed with or without appropriate reasonable accommodation.

<li>Identify and assess the clinical status of potential residents including complete nursing assessment and consultation with current PCP prior to admission.</li>
<li>Secure a statement from resident’s primary care physician indicating any physical or emotional limitations, and certify that the resident is appropriate for the community. </li>
<li>Supervise the medication management program and assisting the residents with self-administration of their medication.</li>
<li>Ensure the coordination of health related services for residents.</li>
<li>Develop a service plan for each resident according to current state regulatory and HallKeen Assisted Living Communities regulations.</li>
<li>Provide ongoing supervision of residents’ health related activities: complying with health care instructions provided by PCP or other health care professionals.</li>
<li>Maintain positive relations with residents, families and physicians.</li>
<li>Report any significant incidents and/or changes in the residents needs to the Executive Director.</li>
<li>Ensure that the Resident Services Nurse evaluates each resident following a hospitalization, incident, accident, injury or illness.</li>
<li>Completion of resident assessments and service plans per regulatory requirements </li>
<li>Supervise all activities of Nursing and Resident Care staff including staffing, scheduling, training and in-services, safety, semi-annual evaluations, and new hire orientation of new staff to ensure safety of residents and clinical competency.</li>
<li>Ensure that community is adequately staffed with appropriate licensed nursing and care staff at all times to safely meet the needs of residents.</li>
<li>Maintain a current contract with a Home Health Agency.</li>
<li>Act as community OSHA Liason. </li>
<li>May perform other duties as assigned.</li>
</ul>
 
Requirements:
Minimum Qualifications

<li>Registered Nurse with a current license in good standing.</li>
<li>Certification or licensed per state guidelines.</li>
<li>Must successfully complete all HallKeen Assisted Living Communities specified training programs.</li>
