About the Role
We are seeking a dedicated and compassionate Physical Therapist to provide personalized, one-on-one care to patients in their homes throughout the beautiful Lake Winnipesaukee region. You'll enjoy the autonomy to assess, evaluate, and implement treatment plans. What You'll Do

<li>Conduct comprehensive patient evaluations and develop individualized treatment plans.</li>
<li>Deliver hands-on therapeutic care tailored to each patient's needs.</li>
<li>Educate patients and families on conditions, treatments, and strategies.</li>
<li>Collaborate with a multidisciplinary team, including physicians and other clinicians.</li>
<li>Provide therapeutic interventions, such as exercises, mobility training, and recommendations for adaptive equipment.</li>
<li>Assess home environments and suggest modifications.</li>
<li>Maintain accurate and timely documentation using MatrixCare EMR.</li>
<li>Actively participate in case conferences and interdisciplinary meetings.</li>

What You Bring
<ul>
<li>Bachelor's degree or higher in Physical Therapy from an accredited program.</li>
<li>Active Physical Therapy license in New Hampshire.</li>
<li>Excellent communication, interpersonal, and problem-solving skills.</li>
<li>Proficiency in EMR systems and comfort with technology.</li>
<li>Experience in home health preferred (we are happy to train passionate clinicians!)</li>
