<li>Manage outstanding Accounts Receivable by reviewing AR aging weekly to identify potential refunds, adjustments or rebilling and communicate with the appropriate biller</li>
<li>Document all collections efforts in PCC</li>
<li>Recommends accounts for outside collection Attorney</li>
<li>Managed Medicaid pending applications timely including coordinating referral to Medicaid Eligibility Specialist or activity working the Medicaid application</li>
<li>Manage annual redeterminations for Medicaid enrollments</li>
<li>Meets with, or ensures that another staff member meets with, all residents/responsible party upon admission and throughout the stay to explain financial obligations and paperwork</li>
<li>Audits new admission files to ensure completeness and accuracy</li>
<li>Meets with, or ensures that another staff member meets with, all residents/responsible party upon discharge to explain any remaining financial obligations</li>
<li>Communicates accounts at risk to the Administrator, prepares for and attends weekly A/R review meetings, documents collection Activities</li>
<li>Attend monthly triple check to ensure Medicare A, Medicare B and Managed Care claims are accurate prior to biller submitting claims</li>
<li>Attend weekly Medicare meeting and weekly MMQ meeting</li>

Elwyn is seeking a Team Leader to join the Program for Assertive Community Treatment (PACT). As the Team Leader, you are the team's clinical and administrative supervisor who also functions as a practicing clinician. The team leader, in conjunction with the team prescriber, is responsible for monitoring each client's clinical status and response to treatment and supervising staff delivery of clinical services to maintain a standard of service excellence and courteous, helpful, and respectful services to program clients.

DUTIES AND RESPONSIBILITIES:
<ul>
<li>Integrates the Prism Model for effective treatment of serious and persistent mental illness into everyday practice and intervention.</li>
<li>Provides person-centered, strength-based, trauma-informed care</li>
<li>Ensures that clients receive effective, understandable, and respectful care that is provided in a manner compatible with client's cultural beliefs and practices and preferred language </li>
<li>Direct day-to-day clinical operations of the PACT team, including scheduling staff work hours to assure appropriate coverage, leading the daily organizational staff meetings and Community Service Plan (tx. plan) meetings; continuously evaluating the status of clients and doing appropriate planning and coordination of treatment activities to ensure immediate attention to changing needs. </li>
<li>Coordinates the admission (identifies/addresses any immediate needs or risks) process and treatment, rehabilitation, and support services of the program in coordination with the prescriber. Schedule the intake, develop and coordinate the Comprehensive Assessment and Community Service Plan; assigns the most appropriate staff to the ITT; overall supervision of the Comprehensive Assessment and Community Service Plan development.</li>
<li>Directs and coordinates the client-centered Comprehensive Assessment of psychiatric history, mental status and diagnosis, physical and dental health, use of substances, education and employment; social development and functioning; activities of daily living; and family structure and relationships </li>
<li>Introduces the client and the family to the team approach of Comprehensive Assessment and the Community Service Plan </li>
<li>Utilizes evidence-based assessment tools that support matching a client's needs to their services </li>
<li>Works with the team to complete a Comprehensive Assessment (within 30 days of admission) as an engagement tool and to understand and respect the client's views of the ways mental illness impacts their life and how they want to be supported in their process of recovery</li>
<li>In partnership with the client and ITT, develops the Community Service Plan within seven days of the team finalizing the comprehensive assessment.</li>
<li>Provides clinical supervision according to ELWYN PACT policy, including field supervision </li>
<li>Provides ongoing assessment of client's symptoms and response to treatment. Make appropriate changes in Community Service Plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior, which puts clients at risk. </li>
<li>Responsible for monitoring the team's training compliance </li>
<li>Participates in staff recruitment, interviewing (1:1 and as a team), on-boarding, orientation, and performance supervision. </li>
<li>Maintains PACT headquarters that are consistent with the PACT model </li>
<li>Monitors the program budget and utilization of program Contingency funds</li>
<li>Oversees the EHR record-keeping compliance in accordance with ELWYN and PACT standards.</li>
<li> Monitors billing and payers, monitor other third-party insurers, and obtain payment from other liable parties for services rendered.</li>
<li>Consistent with PACT standards, maintain adherence to program policies and procedures.</li>
<li>Performs on-call duty for crisis intervention and staff supervision and consultation </li>
<li>Participates in Department of Mental Health contract monitoring meetings </li>
<li>Along with the team, on an ongoing basis, reviews the outcome measures to evaluate community tenure and independence, submitting the report to DMH quarterly. </li>
<li>Leads the team to complete the Tool for Measurement of Assertive Community Treatment (TMACT), with ongoing assessment to adhere to the fidelity of the PACT model</li>
<li>To support the team, provides service coordination for a small group (2-4) clients, including coordination and monitoring the activities of the individual treatment team (ITT); assumes primary responsibility for developing, writing, implementing, evaluating, and revising overall Community Service Plan in collaboration with the client and ITT; provides therapy utilizing evidence-based practices (i.e., CBT-P, CBT, DBT, CAMS, MI), symptom management, ensuring immediate changes are made in the Community Service Plan as clients' needs change; educate and support clients' families, and advocate for clients rights and preferences. </li>
<li>Provides de-escalation techniques to aid the client (s) in managing a crisis, providing intervention to decrease risk, manage symptoms, and maintain the safety</li>
<li>Monitors and maintains electronic card-ex and Daily Organization staff assignment schedules </li>
<li>Leads the team during the daily organizational meeting, offering clinical input and guidance as necessary</li>
<li>Practices a Housing First approach </li>
<li>Performs shift management as needed </li>
<li>Documents client progress, activities, and outreaches in the Electronic Health Record </li>
<li>Ensures the team is utilizing engagement practices across the adult lifespan.</li>

<li>Utilize diagnostic software and tools to identify and resolve complex mechanical and electrical problems.</li>
<li>Maintain accurate service records, documenting all repairs, parts used, and hours worked.</li>
<li>Communicate effectively to explain repairs, provide estimates, and ensure satisfaction.</li>
<li>Manage and maintain a clean and organized service vehicle, ensuring all necessary tools and parts are stocked.</li>
<li>Adhere to all safety regulations and company policies.</li>
<li>Provide exceptional service and represent the company professionally at all times.</li>

What you should know
<ul>
<li>Our top technicians have extensive knowledge in both Electrical and Mechanical Diagnostics.</li>
<li>Preventative Maintenance is a priority, but the ideal candidate should be knowledgeable and proficient in troubleshooting and repair of: HVAC, Braking systems and wheel end components, Engine systems, Emissions and Aftertreatment, and Electrical.</li>
<li>This is a mobile technician; travel is required 100% of the time</li>

What you should have
<ul>
<li>High school diploma or GED required; associate's degree or certificate in diesel technology, heavy equipment repair, or a related field preferred.</li>
<li>3+ years experience as a diesel technician, preferably with mobile service experience.</li>
<li>Strong knowledge of diesel engine systems, hydraulics, electrical systems, and diagnostics.</li>
<li>Proficiency with diagnostic software and tools.</li>
<li>Valid driver's license with a clean driving record</li>
<li>Ability to work independently and manage time effectively in a mobile environment.</li>
<li>Excellent problem-solving and critical-thinking skills.</li>
<li>Strong communication and interpersonal skills.</li>
<li>Ability to lift heavy objects and perform physically demanding tasks in various weather conditions.</li>
<li>Parts and Service management with TMT and TMW </li>
<li>DiagnosticLINK for DD13 motor diagnostics</li>
<li>ToolBox for Wabco Air systems</li>
<li>Service Ranger for Eaton Transmission Diagnostics tools like JPRO or other universal or band specific equipment for electrical diagnostics. </li>
