General Overview:

Who We're Seeking: We are seeking a compassionate and skilled professional to serve as both a Pediatric High-Tech Nurse, delivering comprehensive and family-centered care to children with complex medical needs. In this dual role, you will provide specialized in-home nursing care, operate advanced medical equipment, and manage intricate care plans for medically fragile children. Simultaneously, you will coordinate interdisciplinary teams to offer holistic palliative care, ensuring that every child’s physical, emotional, and spiritual needs are met.

Your Responsibilities: As a member of our dedicated team, you'll:

<li>Deliver exceptional in-home nursing care for pediatric patients requiring advanced medical support.</li>
<li>Operate and manage advanced medical equipment including ventilators, tracheostomies, feeding tubes, and more.</li>
<li>Monitor patients’ conditions and respond promptly to medical emergencies. </li>
<li>Collaborate with families and healthcare teams to create personalized care plans.</li>
<li>Educate and support families in understanding and managing their child’s medical needs.</li>
<li>Facilitate care transitions between hospitals, homes, and other care settings.</li>
<li>Advocate for the needs and preferences of patients and their families withing the healthcare system.</li>

Qualifications:
<ul>
<li>Active Vermont RN license, BSN preferred.</li>
<li>Minimum 2 years of experience in pediatric care, palliative care, or related field. High-tech nursing is preferred.</li>
<li>Strong critical thinking, communication, and organizational skills.</li>
<li>Ability to provide compassionate and culturally sensitive support to families.</li>

Social Worker- One Care

<li>Can you commit to work on a Primary Care Team Model as well as with clients independently to provide appropriate follow-up?</li>

In this role you will:
<ul>
<li>Be responsible for addressing the social service needs of One Care members.</li>
<li>Provide follow-up sessions as needed with One Care health center clients or coordinating services by phone.</li>
<li>Provide brief counseling, advocacy, &amp; resource referrals as needed.</li>
<li>Participate in case conference as needed.</li>
<li>Conduct home visits alone or in conjunction with member(s) of the interdisciplinary team as appropriate, either at the patient’s home or in other locations in the community. </li>
<li>Be an active &amp; integral member of the primary care/BH team. </li>
<li>Advocate for patients with outside agencies such as social service organizations, schools, etc.</li>
<li>Provide crisis intervention as needed.</li>
</ul>
The successful applicant will offer:
<ul>
<li>Human services experience desired.</li>
<li>Ability to work with children &amp; adults, &amp; the elderly.</li>
<li>Minimum of a bachelor’s degree in the human service field (e.g., psychology, sociology) required, BSW preferred; bilingual/bicultural candidates strongly desired.</li>
<li>LSW required</li>
<li>History of good attendance &amp; positive work attitude.</li>
