We are currently hiring a Community Health Worker – C3 Community Partners based in Worcester. This position plays a key role on the care management team, supporting patients with complex or chronic conditions and helping them overcome barriers to care. This position requires flexibility, using both phone and in-person outreach in various settings based on patient and program needs.

In addition, the CHW serves as a care coordinator for assigned Community Partners (CPs), supporting integrated care through team meetings, communication with CPs and providers, and assisting with problem-solving and program coordination.

Essential Functions:

  • Supports C3 ACO providers by encouraging members and caregivers to engage in care management.

  • Helps develop outreach plans and connects individuals and families with needed resources.

  • CP Responsibilities:

  • Serves as the primary contact for Community Partners, builds positive relationships, and makes referrals when appropriate.

  • Builds trusting relationships and uses various outreach methods to engage members in different settings.

  • Addresses cultural and language barriers while coaching members toward personal and clinical goals.

  • Assists with scheduling, SDOH screenings, care coordination, and follow-up needs.

  • Helps members access community services such as Medicaid, food assistance, housing, and transportation.

  • Participates in care team meetings and maintains required documentation.

Required Qualifications:

  • Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
  • Excellent written and oral communication skills.
  • Successful completion of the CHW core competency training within 2 years of employment.
