As a Social Worker with our team, you will:

  • Conductpsychosocial and bereavement assessmentsfor home care and hospice patients and their families, serving as a vital resource.
  • Identify and addresspatient and family needs, including emotional, social, and financial concerns.
  • Developindividualized social work plans of carein collaboration with the family and interdisciplinary team.
  • Providesupport and resourcesto families facing end-of-life care, includingbereavement support and referral information.
  • Connect patients and families withappropriate community agencies and financial resources.
  • Identify and addresspatient and family risk factors, ensuring their safety and well-being.
  • Facilitate effectivefamily and team communication.
  • Providein-service educationto staff and volunteers.
  • Maintaintimely and accurate documentationthat meetsMedicare skilled criteria and medical necessity.
  • Actively participate inteam meetings, in-services, and quality assurance initiatives.
  • Contribute todischarge planningto ensure smooth transitions for patients.



What You Need to Bring:

  • Education:AMaster's degree in Social Work (MSW)
    from a Council on Social Work Education (CSWE) accredited institution.
  • Experience:Previous social work experienceis highly valued.
  • Communication Skills:Excellent verbal and writtencommunication skillsare essential for effective collaboration and patient interaction.
  • Compliance:Must successfully pass requiredcriminal and driving record checks, as well as aBureau of Elderly and Adult Services check.
  • Transportation:Reliable transportation, avalid driver's license, and proof ofcar insurancewith $100,000/$300,000 coverage are required due to frequent local travel.
