Available Shifts - Job #/Schedule (Please include job number with application):

  • #1164 - Every other Monday, 10am-1pm, 3-4 hrs Support staff needed for young man living in Attleboro area. Join him while accessing his community. Support staff will assist in identifying/obtaining/maintaining meaningful volunteer opportunities in the community as well as appropriate social opportunities.
  • #1247 - Monday and Tuesdays 3pm-5pm, Fridays 10am-12pm, 4-6 hrs/wk Support staff needed for young lady living in the Attleboro area. Staff should be self-motivated and enthusiastic. Looking for staff to assist with community access, safety skills, and development of new work skills. Assist in maintaining an established schedule. Experience working with ASD population needed.

General Description

A Community Based Direct Support Professional works in the Family Support Department within Agency with Choice, Individualized Home Supports, and Adult Companion Services. Staff works in conjunction with supported individuals to understand their strengths, needs, and desires for services. Staff’s role is to assist the individual’s participation in normative home and community-based activities, centering on the individual’s person-centered goals. Goals focus on maintaining and increasing independence; encouraging self advocacy skills including helping individuals to make their own choices, free from judgement; building on community connections; encouraging good health and wellness; and forming positive and healthy relationships. Staff report directly to Family Support or AWC Coordinator.

Job Responsibilities:

  • Works with individual on set goals and objectives under chosen services
  • Works, as appropriate, with the family in implementing chosen goals
  • Completes established documentation for reporting on goals after each shift with individual and follows protocol for submitting this documentation
  • Supports individuals in achieving and maintaining personal well-being and safety
  • Completes timesheet, obtains family signature for verification of work, and submits according to due date schedule
  • Implements Positive Behavioral Supports
  • Treats individuals/family with dignity and respect
  • Provides transportation as required
  • Supports individuals in achieving and maintaining personal well-being and safety
  • All other job related duties as assigned by supervisor or Director.

Minimum Qualifications:

High school diploma or GED is required, and at least one year of relevant experience in working on skills development with individuals with intellectual disabilities or Autism is preferred. Additional qualifications may apply for certain individuals based on their specific needs.
