As Director of Social Services, you will lead our Social Services department and play a pivotal role in ensuring residents' emotional, social, and psychological needs are met. You will work collaboratively with the interdisciplinary care team and provide critical support to residents during transitions, life events, and care planning.

Key Responsibilities

• Lead and manage the Social Services department, ensuring compliance with federal, state, and facility guidelines.
• Conduct and oversee psychosocial assessments and individualized care planning.
• Provide support and counseling to residents and families from admission through discharge.
• Coordinate care plans in interdisciplinary meetings and ensure timely documentation.
• Assist with discharge planning, including PASRR evaluations and community referrals.
• Advocate for resident rights and connect families with appropriate external resources.
• Maintain confidentiality and promote dignity and respect in all resident interactions.

Qualifications

• Massachusetts licensure as LSW, LCSW, LICSW, or LSWA required.
• Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred.
• Experience in long-term care or nursing home setting strongly preferred.
• Familiarity with MDS 3.0, PASRR evaluations, and care planning processes is highly desirable.
• Proven leadership skills with excellent communication and interpersonal abilities.
• Compassionate, organized, and dedicated to resident advocacy and team collaboration.
