We are currently hiring a Community Health Worker – C3 Care Management based in Worcester. As part of the care management team, the Community Health Worker (CHW) supports individuals with complex or chronic conditions who face barriers to care. The role is flexible and changes day-to-day to meet members’ needs. Outreach may be done by phone or in person in various settings, including the community, homes, facilities, or the health center.

Essential Functions:

  • Engages members and caregivers in care management under guidance of C3 ACO providers, developing outreach plans and using various methods to connect with individuals and groups.

  • Builds trusting relationships with members, families, and care teams; shares resources, referrals, and community insights to support health goals and improvement efforts.

  • Gathers and combines information from multiple sources to better understand clients, their families, and their community needs.

  • Addresses barriers such as language or cultural challenges, and coaches members toward personal and clinical goals.

  • Assists with scheduling appointments, completing SDOH screenings, coordinating care transitions, supporting referrals, and ensuring appropriate follow-up.

  • Helps members access community and government programs, including completing paperwork for benefits such as Medicaid, food and nutrition services, housing, and transportation.

  • Participates in integrated care team meetings and maintains required documentation, reporting, and record-keeping.

Required Qualifications:

  • Minimum 1 year of work or volunteer experience in human services or healthcare with demonstrated knowledge of or ability to work with the targeted community.
  • Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
  • Excellent written and oral communication skills.
  • Successful completion of the CHW core competency training within 2 years of employment.
