In this role, you’ll provide direct support, manage services, and supervise team members to ensure that every person we serve receives consistent, high-quality, and person-centered care.
What You’ll Do:
- Support and coordinate services for individuals with developmental disabilities, ensuring their goals, preferences, and needs are reflected in every aspect of their support.
- Develop and implement ISP goals, ensuring documentation and follow-through in collaboration with each person’s support team.
- Advocate and communicate regularly with families, guardians, DDS, healthcare providers, and community partners.
- Respond to emergencies and provide on-call support as needed.
- Ensure compliance with state regulations and CHS policies, including safety, health, medication administration, and documentation standards.
- Promote community inclusion by developing natural supports and helping individuals access medical, dental, and wellness services.
- Participate in quality initiatives, team meetings, and continuous improvement efforts across programs.

Requirements:What You Bring:
- Associate’s Degree in a related field required; Bachelor’s Degree preferred.
- Minimum of 2 years’ experience supporting individuals with developmental disabilities.
- At least 1 year of supervisory experience in human services.
- Strong documentation and communication skills.
- Ability to multitask, problem-solve, and lead with compassion and professionalism.
- Valid driver’s license (MA, NH, RI) and reliable transportation required.
- Must be able to lift 30 lbs and meet the physical demands of the role.
