We are seeking a Medical Social Worker (MSW) to conduct psychosocial assessments and support patients and families receiving Home Care and Hospice services in the community. As part of the interdisciplinary team, the MSW acts as a resource, helping patients navigate illness, long-term care planning, and community resources.

Work hours are flexible, with options for full-time or part-time schedules, primarily during day shifts.
Responsibilities involve psychosocial assessments, social evaluations, resource referrals, discharge planning, and collaboration with healthcare team members.
Qualifications include a Master’s in social work, healthcare experience, strong communication skills, valid driver’s license, insurance, and recent physical and TB tests.
