Upper Valley Services (UVS) is one of Vermonts designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.
UVS is seeking a compassionate and skilled team member toensure quality care of the individuals residing at one of our residential locations by managing and coordinating services
and staffing that support their health, safety and wellbeing, personal interests, and goals through
collaboration with Service Coordinator(s), Program Director, Associate Program Director, and UVS
nursing. Additionally, this role is responsible for supervision of the program staff which includes but is
not limited to development of shift schedules, oversight of training, oversight of medication refills and
grocery needs, documentation, menus for regular therapeutic diets, and ensuring maintenance and
cleanliness of the homes.
Essential Functions:
Assure the safety and well-being of the Individuals in the home
Supervise and support the program staff through guidance, role modeling, and teaching, in addition
to maintaining a strong and collaborative program team
Ensure the overall quality of the homes atmosphere for the Individuals
Ensure proper medication administration, management, and documentation
Ensure medication refills are filled timely
Maintain flexibility in work schedule and ensure coverage when short staffed and/or in emergency
situations
Schedule
Be present in the home an average of 32 hours per week (4 days on site), which will
include 2 full (8-12 hr) shifts with each individual, supervision with staff and may include
trips to obtain groceries, medications, or Moretown office needs. 8 hours per week may be
conducted remotely to manage additional documentation, supervision, coordination and
timesheet needs.
Education and Experience:
Bachelors degree, preferred in Human Services or related field with at least 2 years of experience
working with individuals with developmental disabilities and minimum of one year supervisory
experience OR a minimum of 3 years experience working with individuals with developmental
disabilities and a minimum of 1 year of supervisory experience. Medical background preferred but
not required (LNA or LPN). Experience supporting individuals with medical and personal care
needs preferred.

Travel Requirements:
Valid drivers license, access to reliable transportation, and insurance meeting the Agencys
standards
