<li>Greets patients and escorts them from the waiting room to exam rooms, prepares patient for exam or procedure, completes rooming and patient intake, provides patient education</li>
<li>Assists provider during exam and office procedures including ultrasound, injections, specimen draws, casting and brace fittings and removal, impact/concussion testing per protocol</li>
<li>Prepares injections, blood draws per protocol, preps samples for pick up by lab</li>
<li>Prepares and maintains supplies and equipment, including medications and sterilizations, following OSHA, CLIA and practice protocols</li>
<li>Answers all incoming calls with clear and pleasant speaking voice, giving information for all routine questions (ex: office hours, fax number), and directs calls to the appropriate individual or team for handling.</li>
<li>Prepares work/school notes and prescription refill requests per practice and MassPAT protocols</li>
<li>Assists patient with dressing, ambulation/ transport and care needs</li>
<li>Imports labs and imaging discs, documents visits and orders, takes messages using eCW</li>

<li>Maintains work queues throughout the day following protocol</li>
<li>Cross covers other clinical areas as needed</li>

Requirements:

A community based Personal Care Management (PCM) Skills Trainer for our Personal Care Attendant Program will educate consumers on how to successfully manage their consumer-directed hands-on care and also be primarily responsible for other skills training department wide tasks and processes.
This Personal Care Management Skills Trainer will cover Suffolk County area in Massachusetts and up to a 60 mile radius of your home address.

 Essential Job Functions
<ul>
<li>Follow-up on assigned referrals in a timely manner as determined by contract.</li>
<li>Assess consumers’ ability to manage PCA services.</li>
<li>Train consumers and/or surrogates the skills needed to manage their PCA program as outlined in the Mass Health Service Agreement and according to the Personal Care Management (PCM) Contract.</li>
<li>Maintain confidentiality in all consumer related issues.</li>
<li>Attend in-service, supervision and staff development meetings when requested by manager.</li>
<li>Demonstrate a working knowledge of program policies and procedures and Mass Health regulations.</li>
<li>Demonstrate a commitment to the Independent Living philosophy of consumer control.</li>
<li>Must report all suspected incidents of consumer sexual/physical abuse and neglect to the Disabled Person Protection Commission (DPPC).</li>
<li>Work within company policies and in accordance with the PCM Contract in order to complete and track for receipt of required documentation within required timelines:</li>
<li>Assigned referrals and intake documentation</li>
<li>Service Agreements and Assessments for re-evaluations</li>
<li>Supportive documentation for intakes and re-evaluations as needed</li>
<li>Return all phone calls from surrogates and consumers within 48 hours.</li>
<li>Work within policies to complete and submit internal requirements:</li>
<li>Progress Notes</li>
<li>Dayforce, Outlook Schedule</li>
<li>Mileage and Expense Forms</li>
<li>Follow up with Manager and/or Program Assistant on requests for skills training and tracking reports:

<li>Quarterly and Annual Reviews</li>

 Qualifications
<ul>
<li>Experience providing services for persons with disabilities and knowledge of PCA programs is preferred.</li>
<li>Experience or education in teaching or counseling is helpful.</li>
<li>Knowledge of community resources and social service systems is beneficial.</li>
<li>Excellent communication, organizational, and writing skills.</li>
<li>Basic computer skills in Microsoft Office/Outlook and the ability to navigate databases.</li>
<li>Specialized training provided as needed.</li>
</ul>
