Summary: The Developmental Specialist 3 will provide support and assistance in Level 3 day habilitation programs. Staff will provide support to individuals who need daily intervention, supervision, and skills training in activities of daily living within a day program environment and in the community. Individuals may require moderate to considerable physical assistance or accommodation. Staff will receive specialized training to provide enhanced supervision or interventions designed to address multiple disabilities, including behavioral supports.

Essential Duties and Responsibilities: 
<ol>
<li>Provide services to individuals according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards as well as Bridgewell policies and procedures.</li>
<li>Plan and implement an individualized program, based on each individual's needs following their Day Habilitation Service Plan.</li>
<li>Supervise the implementation of the Day Habilitation Service Plan and maintain the individual records in eHana.</li>
<li>Administer and interpret assessments to determine each individual's skill level on an annual basis.</li>
<li>Teach skills to individuals and small groups for training in designated areas.</li>
<li>Develop adaptive equipment and jigs to ensure individuals attain goals.</li>
<li>Assist people in making decisions about their daily routine/activities.</li>
<li>Assure that people have access to communication with family, friends, and significant others.</li>
<li>Ensure participation in community events, including attendance at volunteer sites, libraries, YMCAs, and any particular place of interest to them.</li>
<li>Assist with the upkeep and maintenance of the program, follow the cleaning schedule, report any maintenance issues, etc.</li>

Required Education/Experience:

MHA’s Integration &amp; Community Living (ICL) program supports individuals with developmental disabilities and brain injuries in living independently and meaningfully in the community. In partnership with the Department of Developmental Services (DDS), ICL provides person-centered residential and outreach services throughout Western Massachusetts.

Position Summary
As a Residential Support Specialist, you will provide a supportive and safe environment in community residential settings within MHA’s ICL (Integrated Community Living) department. This role involves traveling to different program locations based on staffing needs, offering flexibility and consistent support across the department. You will help foster growth, independence, and community engagement for individuals who have experienced a brain injury resulting in physical limitations, mental health, and/or cognitive challenges. 
Pay Rate: $20 an hour
Open Shift: Sundays 9am-3pm &amp; 3pm-11pm (14h)
Key Responsibilities
<ul>
<li>Provide respectful, person-centered support to participants in daily living activities, personal care, and skill development.</li>
<li>Promote community integration by facilitating access to transportation, social activities, and external resources.</li>
<li>Support participants' health needs, including medication administration (per MAP standards), appointment scheduling, and monitoring overall wellbeing.</li>
<li>Assist with financial skills and money management, maintaining accurate documentation of all expenditures.</li>
<li>Respond to emergencies and crisis situations appropriately, following agency protocols and communicating with supervisors.</li>
<li>Advocate for participants and help them build self-advocacy skills; serve as a liaison with families, providers, and community supports.</li>
<li>Contribute to the development and implementation of Individual Service Plans (ISPs), documenting progress and participation.</li>
<li>Maintain accurate and timely documentation, including daily logs, incident reports, and health records.</li>
<li>Promote a safe environment by following all safety procedures, assisting with emergency drills, and identifying potential hazards.</li>
<li>Collaborate effectively with team members, attend training, maintain certifications (MAP, CPR, First Aid), and actively participate in meetings and supervision.</li>

Job Summary:
The Resident Care Director Assistant is responsible for evaluating and assessing the general 
Health functioning of all residents, and designing and revises as needed the care plan for each resident. This position ensures compliance with all applicable Federal and State regulations. 
 
Duties and Responsibilities:
The following essential functions are the fundamental job duties of the position to be completed with or without appropriate reasonable accommodation.

<li>Secure a statement from resident’s primary care physician indicating any physical or emotional limitations, and certify that the resident is appropriate for the community. </li>
<li>Assist in the supervise the medication management program and assisting the residents with self-administration of their medication.</li>
<li>Ensure the coordination of health related services for residents.</li>
<li>Assist in the development of service plans for each resident according to State regulations and HallKeen Assisted Living Communities policies.</li>
<li>Provide ongoing supervision of residents’ health related activities: complying with health care instructions provided by PCP or other health care professionals.</li>
<li>Maintain positive relations with residents, families and physicians.</li>
<li>Report any significant incidents and/or changes in the residents needs to the Executive Director.</li>
<li>Ensure that each resident is reevaluated following a hospitalization, incident, accident, injury or illness.</li>
<li>Assist in the completion of resident assessments and service plans per state regulations </li>
<li>Supervise all activities of Resident Care staff including staffing, scheduling, training and in-services, safety, semi-annual evaluations and new hire orientation of new staff.</li>
<li>Complete all RCA and Wellness nurse schedules according to need and staffing plan.</li>
<li>May perform other duties as assigned.</li>

Requirements:
Minimum Qualifications:· 
Must be licensed in appropriate State as LPN, RN Preferred· Must successfully complete all HallKeen Assisted Living Communities specified training programs.· Able to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Able to deal with problems involving a few concrete variables in standardized situations.· Able to work various schedules and shifts as needed.· Must have a solid understanding of the Wellness department, Policies and Procedures, regulation requirements.
