<li>Provides regular, mandatory trainings for all Agency staff including, but not limited to, fire safety, emergency procedures, universal precautions, vital signs, assessing signs and symptoms of illness or injury and other relevant trainings.</li>
<li>Reviews and finalizes reporting including incident reports, restraint reports, medication occurrence reports, ISP assessments and objectives and other key reports.</li>
<li>Ensures Agency is compliant with state regulatory standards, as well as, standards set forth by the Commission on Accreditation of Rehabilitation Facilities (CARF).</li>
<li>Provides rotational weekend and evening coverage for emergency on-call.</li>

Essential Duties and Responsibilities:
<ol>
<li>Ensure program, agency and accreditation standards are met regarding the quality of care including individual behavior and treatment plans, ISP documentation, HCSIS, and other documentation as required, and in line with Bridgewell Policies &amp; Procedures.</li>
<li>Ensure the staffing needs of the program are met based on defined ratios and attendance of persons served. This includes coordination of work schedules, hiring of new staff, and coordinating the use of per-diem and agency relief staff, as needed.</li>
<li>Positively manage staff by conducting monthly supervisions with each staff and team meetings, supporting, coaching and mentoring staff to develop skills and abilities, and providing recognition opportunities for staff. </li>
<li>Provide timely Performance Evaluations. Ensure performance standards are met, and proactively address performance concerns, working with program management and Human Resources, as appropriate.</li>
<li>Ensure self and employees maintain certifications (such as CPR/1st Aid, MAP, PPS and PABC, as required) and complete all other training requirements of Bridgewell and the program's funding sources.</li>
<li>Ensure administrative duties are completed and followed as required in areas including finance, human resources, payroll, quality assurance, electronic health records and Medication Administration Procedures.</li>
<li>Responsible for On-Call coverage based on a rotating schedule for that program, and/or cluster, as designated. On-Call coverage includes addressing concerns and providing directions to employees, managing callouts and scheduling per diem staff, on-call reporting, as well as possible coverage of shifts if staffing cannot be coordinated for a shift.</li>
<li>Work collaboratively with clinical and nursing teams to ensure the behavioral and medical needs of the people served are met. Collaborate with all service providers, ensuring communication and documentation is clear regarding the coordination and management of each person's needs.</li>
<li>Work with guardians and family members on issues related to the care and services provided to their family members. Communicate proactively, as appropriate, working to build a positive relationship, and ensuring staff are also trained in the best practices of working with families.</li>
<li>Manage the financial responsibilities of the program, including program expenses, monitoring overtime, supply ordering and monthly budget review. </li>
<li>Ensure people served are provided support with financial needs, such as managing funds, applying for food stamps, social security, Mass Health, housing vouchers/section 8 applications and/or employment.</li>
<li>Provide direct care in the absence of program staff, as needed. This can include covering individual shifts, as well as, assisting with management needs in other programs where either a management vacancy or extended absence is occurring.</li>
<li>Coordinate or assist with program community outings and meetings with families. Ensure all information and materials required for any meeting or outing are adequately prepared, and that positive relationships are built and maintained. Ensure positive neighbor relationships are maintained, and that any concerns are raised to senior management.</li>
<li>Manage the technology needs of the program, including employee time and attendance, and data management/electronic health record systems. Ensure information is tracked as required, and system needs and issues are raised to the IT Department or Payroll Department, as appropriate.</li>
<li>Ensure vehicles assigned to the program maintain registrations as required and receive maintenance as scheduled. Coordinate and address needs with the Transportation Department. Ensure employee use of the vans follow established Bridgewell protocols. Report accidents in a timely manner to Transportation.</li>
<li>Ensure the confidentiality of Personal Health Information, records of people served, and other related information is in compliance with HIPAA, and other state or federal mandates.</li>
<li>Ensure safe operations of the program by providing safety training per the Safety Calendar, addressing program physical maintenance needs, conducting audits as required, and alerting management to any concerns.</li>

Requirements:
Minimum Qualifications
<ul>
<li>Must have current certification as a nursing assistant or have successfully completed the certified home health aide or homemaker/personal care homemaker training program.</li>

<li>Must successfully complete HallKeen Assisted Living Communities, LLC’s Medication Training.</li>
<li>Able to read and comprehend simple instructions, short correspondence, and memos. Able to write simple correspondence. Able to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.</li>
<li>Able to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.</li>
<li>Able to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.</li>

Summary: The Direct Support Professional will provide support and assistance to people with developmental, physical and psychiatric disabilities as needed based on the needs of the residential program assigned. The Direct Support Professional will provide support to people in our services who need daily intervention, supervision and skills training in activities of daily living while managing within a home environment and in the community. People in our services may require physical assistance or accommodation. Staff will receive and must pass specialized training to provide enhanced supervision or interventions designed to address multiple disabilities which may include behavioral support or enhanced medical care. Staff may be required to have specialized certifications; be credentialed or trained in specialized areas such as PABC or ABA techniques; and/or have advanced training in areas such as autism, behavioral support and/or healthcare.
Schedule: Sunday 5:00PM-11:00PM, Monday 3:00PM-11:00PM, Saturday 9:00AM-5:00PM (22 hours)
Essential Duties and Responsibilities: 
<ol>
<li>Provide support and assist people in our services with skills training in all activities of daily living during the day, evening and/or overnight hours (depending on the shift the employee is assigned).</li>
<li>Create opportunities and provide support to people in our services to make decisions to develop their skills and foster independence.</li>
<li>Provide services to people in our services according to DDS, DMH, MassHealth, HUD, DPH, CARF and all applicable regulatory standards, as well as Bridgewell policies and procedures.</li>
<li>Provide support and assist people in our services with all medical, clinical and financial needs.</li>
<li>Transport and accompany people in our services as needed to and from medical, recreational and other activities outside the program.</li>
<li>Maintain daily progress notes, shift checklists, shift assignments and other required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF and all applicable regulatory standards as well as Bridgewell policies and procedures.</li>
<li>Complete household cleaning duties as assigned.</li>
<li>Conduct all steady care calls as per guidelines (applies to all awake overnight shifts)</li>
<li>Assist people in our services with accessing resources and recreational opportunities in the community.</li>
<li>Administer medications according to MAP regulations, as applicable.</li>
<li>Maintain all mandatory training certifications such as FA/CPR, PABC and MAP (not all all-inclusive list). </li>
<li>Maintain additional mandated certifications and trainings as required by the program and Bridgewell policies and procedures.</li>
<li>Develop positive relationships with guardians, family members and other stakeholders.</li>
<li>Serve as the program's specific Fire Safety Officer or Safety Coordinator or Human Rights Officer. </li>
<li>Comply with Bridgewell Safety Checks Policy.</li>
<li>Attend and participate in program staff meetings and supervision meetings.</li>

Required Education/Experience:

Physical Demands:
The physical demands of employees working in residential programs assisting people with their care needs are varied and require the employee to be able to maneuver people of varying weights, and provide lifting, balanced support and physical intervention in a few scenarios.
