Job Description
Deliver quality general/psychiatric nursing care to patients who are hospitalized for treatment of psychiatric or emotional disorders. Knows and applies current nursing theory, principles of care and nursing procedures in performing the following functions appropriate to the age of the patient, maintains a safe and clean care environment, quality improvement, assist in the supervision of the Behavioral Health Technician, actively and consistently contributes to department operations and communications, behaves in a manner consistent with the mission and objectives of Aurora Behavioral Health System.

Duties:

<li>Assessments are completed with treatment referral recommendations made and documented, using the approved admission criteria and make decisions concerning the level of care for treatment using the least restrictive level appropriate. The plan for episode of care is initiated at admission to all levels of care. </li>
<li>Assists clinicians, administration and physician in the assessment of pending admits. </li>
<li>Ensures admission paperwork is completed in accordance with hospital policy </li>
<li>Plans, coordinates, and evaluates direct and indirect facility assessments and client admissions.</li>
<li>Demonstrates the ability to provide and assure the carrying out of established nursing procedures utilizing the nursing process, and assuring the maintenance of a safe clean environment. Provides age appropriate total patient care demonstrating the ability to assess patient and situation needs and problems. </li>
<li>Assist medical provider’s with rounds, treatments providing timely noting and implementation of medical provider orders </li>
<li>Demonstrates thorough, concise and timely documentation, shift reports and verbal communications. Communicates pertinent patient information and unit information to medical providers, House Supervisor, and other appropriate persons while maintaining client confidentiality. </li>
<li>Assists in the coordination of services with medical staff, nursing staff, and other departments. Responds rapidly and effectively during emergency situations. </li>
<li>Maintains own clinical skills staying current in use of equipment, procedures, and trends in nursing. Efficiently and effectively provides total patient care including assessment, teaching, care planning, rehabilitation, medication, treatments, activities of daily living, discharge planning, etc. Completes annual competencies. </li>
<li>Assist in the orientation of new personnel </li>
<li>Serves on committees and attends meetings actively participating. Follows and assists in the implementation of policies and procedures </li>
<li>Assists in monitoring, procurement and proper inventory of supplies. Assists in maintaining equipment. </li>
<li>Self motivated and able to make decision independently keeping House Supervisor/Unit Manager and others informed. Able to prioritize and manage time efficiently. </li>

Essential Duties and Responsibilities:
<ul>
<li>Provide direct care coverage and support for individuals served, responding to individual issues during assigned shifts.</li>
<li>Work with individuals to promote growth in each of QUEST's quality of life areas (i.e., Individual Control, Respect and Dignity, Relationships, Community Membership, Personal Growth &amp; Accomplishments, Personal well-being). </li>
<li>Assist individuals in working on areas identified in their Individual Support Plans (in conjunction with the efforts of other team members).</li>
</ul>
<ul>
<li>Support individuals as needed or directed, in the areas of problem solving, personal care, medical care, socialization and leisure activities, interpersonal skills, household, money management, activities of daily living, asserting their human and civil rights.</li>
<li>Assist in the development and implementation of behavior support plans. </li>
<li>Plans in conjunction with the efforts of other team members.</li>
<li>Know emergency procedures and basic emergency facts regarding all individuals living at the location where coverage is being provided.</li>
<li>Complete any necessary documentation, including (but not limited to) Communication Log, Case Log, ROADs Charts, Daily Progress notes, Incident Reports, Medication and Treatment Charts, Medication Occurrence Reports.</li>
<li>If Certified to Dispense medications as prescribed. Report any medication occurrences to the Program Manager, complete necessary forms and follow applicable procedures.</li>
<li>Transport and/or accompany individuals to appointments and other community-based activities and events as needed.</li>
<li>Maintain Triangle property in good and sanitary condition.</li>
<li>Attend and participate in supervision and staff meetings as scheduled by the Program Manager.</li>
<li>Attending training as directed by the Program Manager.</li>
<li>Additional duties as assigned by the Program Manager.</li>
<li>Accomplishes all tasks as appropriate.</li>
</ul>
Qualifications
<ul>
<li> High school diploma or general education degree (GED), one to three months related experience and/or training, or equivalent combination of education and experience.</li>
</ul>
Language Skills
<ul>
<li>Ability to verbally communicate, read, analyze, interpret, and write documents. </li>
<li>Ability to respond and navigate common inquiries or complaints from regulatory agencies, or family members. </li>
