Community Bridges is looking for 2 full-time Direct Support Professional in Franklin, NH! 
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.

In the Behavioral Day Program, our Direct Support Professionals support those who experience developmental disability and occasional behavioral challenges in their home and community. Each person regular daily routines and activities they enjoy in their community and they love to learn and see new things. The Direct Support Professionals provides support to the individuals by attending medical appointments, monitoring their health, safety, nutrition, grocery shopping, community activities, and community connections.

Qualifications:

<li>Education: High School Diploma or GED required.</li>
<li>Experience: Experience in the human service or related field with persons experiencing disabilities desirable.</li>

Compensation and Benefits:

<li>Complete documentation requirements as specified by the Individual Service Plan to include the scheduled data collection and monthly progress notes to show an increase or decrease in behaviors to continually support individuals as they grow and reach goals.</li>
<li>Assist in the orientation/training of new staff, which may include introduction to an individual's support persons and to an individual's ISP, strategies, dreams and goals, and treatment plan.</li>
<li>Distribute medication in accordance with He-m1201 as applicable.</li>

Provides individuals in community-based program with support services designed to promote the independence, productivity, integration and inclusion in community. This position implements programs aimed at teaching new skills in the areas of self-care, managing behavior, community integration and other activities of daily living. This is carried out with respect and dignity and in accordance with Walnut Street Inc.(WSC) policies and procedures. 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 
Direct Support Skills
<ul>
<li>Follow age and population specific recommendations. Understands and applies competencies specific to the general characteristics of each age group and specific population served</li>
<li>Activities of Daily Living Assists individuals with Activities of Daily living which includes but is not limited to; personal hygiene/self-care tasks, dressing.</li>
<li>Provides therapeutic environment. Provides supervision to insure safety and well- being of individuals. . Provides supports that promote independence through skill building programs. Encourages participation and is helpful, supportive and respectful of differences with regards to unique needs, age, gender and culture of the individual being supported.</li>

The primary purpose of this position is to provide each of the assigned residents with routine daily care by assisting with direct resident care duties and indirect resident care duties, as directed by the charge nurse, and/or certified nursing assistants. 
Assist residents in dressing and personal hygiene. Assist CNA’s with residents requiring 2-staff transfer except in mechanical transfers. Make occupied bed. Assist in obtaining weights. Provide full or partial bed bath. Perform incontinent care in bed to residents who can move in bed. Assist residents who are not on swallowing or aspiration risk with meals. Pass, set up, and collect trays. Place belongings within resident’s reach (call light, glasses, TV remote, glasses, etc.) Answers call bells in a timely manner. Make unoccupied bed. Empty commodes and urinals. Transport residents to the dining room for meals. Accompanying residents on outside appointments as needed. Visit with residents providing 1:1 supportive interaction (i.e., read their mail, newspapers, or books to them, transport them for visits outdoors when within residents’ care plan, transport to activities, transport to hairdresser, help to write letters, make telephone calls, etc.) Assist residents in communication involving technology (zoom, face time, skype, google meet, etc.). Assist with monitoring residents to maintain safety providing 1:1 supervision as needed. Organize, straighten, and clean closets and dressers (i.e., clothes hung neatly on hangers in closet, folded neatly in drawers, trash and soiled laundry removed.) Report resident’s intake to Team Leaders. Offer the resident nourishments and/or distribute nourishments from the dietary department. Check fall precautions: bed monitor, mats, and personal alarms. Check and clean dirty wheelchairs after each meal or after incontinent episode. Check and clean tub and shower room. Greet newly admitted residents upon admission, escort to room as necessary, receive paperwork from referring agency, and inform Unit Manager/Charge Nurse of arrival. Assist with inventory of new admissions. Assist in arranging for and making appointments for diagnostic therapeutic services. Direct visitors to residents' rooms, office areas, etc. Photocopy and fax per the needs of the unit.

<li>Maintaining a clean, safe, and healthy environment within clients residence. Duties may include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing the client's dishes or laundry, and other tasks to maintain cleanliness and safety for the client.</li>
<li>The performance of all personal care activities contained in a written Plan of Care which may include assisting the client with personal hygiene, ambulation and exercise, eating, dressing, shaving, physical transfer, and other duties as assigned.</li>
<li>Assisting clients with running errands such as doctor's appointments, grocery shopping and any approved activities to improve quality of life.</li>
<li>Providing nutritional support as needed. Such as meal preparation and assisting with feeding.</li>
<li>Document and report any changes in client's condition to the care management team, included but not limited to appearance and gross behavioral changes in the client.</li>
<li>The caregiver may also provide medication reminders.</li>
<li>Abides by the agencys infection control policies, including proper handwashing techniques consistent with CDC Guidelines.</li>
