Walking and standing are required frequently with occasional stair climbing. Must be able to push wheelchairs and operate wheelchair lifts/Hoyer and barrier--free lifts. <li>Assists with G-Tubes, catheters, medication administration, oxygen use, aspiration and other medical needs.</li>
<li>Assesses pain and discomfort in non-verbal individuals.</li>
<li>Develop a relationship map to gather information about the individual in order to establish social inclusion opportunities.</li>
<li>Assist individuals in developing a network of people in their lives. Maximize opportunities for the individuals they support with disabilities to get to know others and maximize the opportunities for others to get to know the individual.</li>
<li>Assist the individual in obtaining a sense of self-esteem through active participation in the community.</li>
<li>Provide individualized type and level of support to include, but not be limited to, personal care and/or skill acquisition etc.</li>
<li>Assist individuals in establishing, maintaining, and presenting a positive image. This may involve such activities as assisting in the bathroom, bathing, showering, and dental hygiene, etc.</li>
<li>Assist in developing and individualizing the yearly Person Centered Planning by providing information for assessments and by developing implementation strategies for goals/objectives and methods for monitoring the delivery and quality of supports to the individual.</li>
<li>Complete documentation requirements as specified by the Individual Service Plan to include the scheduled data collection and monthly progress notes.</li>
<li>Assist in the orientation/training of new staff, which may include introduction to an individual's support persons and to an individual's ISP, strategies, dreams and goals, and treatment plan.</li>
<li>Distribute medication in accordance with He-m1201 as applicable.</li>
<li>Report any safety, health or human rights concern immediately to the Supervisor.</li>
<li>Understand and ensure compliance with applicable state regulations.</li>
<li>Perform other duties and responsibilities as assigned.</li>

Work Environment and Physical Demands:
<ul>
<li>Work environment- The positions is typically indoors, unless the individual wants to go outside. The noise level is minimal at the residence.</li>
<li>Physical demands- Direct Support Providers must be physically able to lift up to 50 lbs. and to provide transfers as necessary. Walking and standing required frequently with occasional stair climbing. Must be able to push wheelchairs and operate wheelchair lifts/Hoyer and barrier free lifts. Emergency behavioral intervention must be followed in accordance with behavioral intervention techniques and individual behavioral plans.</li>

Travel: 
Must have access to personal vehicle during all scheduled work hours. Frequently travels with supported individuals utilizing personal vehicle as well Community Choices vehicles. Must possess and maintain a valid NH license and safe driver's record in accordance with Community Choices standards with adequate liability insurance.

Summary: The Direct Support Professional 2 will support and assist in Level 2 residential programs. Staff will help individuals who need daily intervention, supervision, and skills training in activities of daily living while managing within a home environment and in the community. Individuals may require moderate physical assistance or accommodation. Staff will receive and must pass specialized training to provide enhanced supervision or interventions designed to address multiple disabilities, including behavioral support or enhanced medical care.
Schedule: Saturday and Sunday 9:00AM-5:00PM (16 hours)
Essential Duties and Responsibilities: 
<ol>
<li>Provide support and assist individuals with skills training in all activities of daily living.</li>
<li>Create opportunities and provide support to individuals to make decisions to develop their skills and foster independence.</li>
<li>Provide services to individuals according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards, as well as Bridgewell policies and procedures.</li>
<li>Provide support and assist individuals with all medical, clinical, and financial needs.</li>
<li>Transport and accompany individuals as needed to and from medical, recreational, and other activities outside the program.</li>
<li>Maintain required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards as well as Bridgewell policies and procedures.</li>
<li>Assist individuals with accessing resources and recreational opportunities in the community.</li>
<li>Administer medications according to MAP regulations, as applicable.</li>
<li>Maintain core training certifications including FA/CPR, PABC, and MAP.</li>
<li>Maintain other additional required training and certifications as required by the program.</li>
<li>Develop positive relationships with guardians, family members, and other stakeholders.</li>
<li>Other duties as assigned.</li>
</ol>
Required Education/Experience: 
<ol>
<li>Must have a high school diploma or GED. Associates or DSP Certification preferred.</li>
<li>A minimum of 1 year of experience working with people with developmental or psychiatric disabilities is preferred.</li>
<li>Bachelor's degree in a related field is a plus.</li>
</ol>
Required Skills/Knowledge: 
<ol>
<li>Excellent written and oral communication skills so that the writing of required documents is clear and articulate.</li>
<li>Excellent organizational and problem-solving skills.</li>
<li>Valid driver's license and one year of driving experience.</li>
<li>Computer literacy includes the ability to use proficiently Microsoft products and web-based applications used by the agency.</li>
