We are currently looking for 2 part-time Life Skills Coaches for the Community Support Services Department in Concord, NH!

The Life Skills Coaches provide coaching and assistance to adults with intellectual disabilities. This includes assisting with skill development on activities of daily living which may include supports with money management and budgeting, meal planning and preparation, health maintenance and expanding knowledge of community resources. Some other activities could include assisting with shopping; coaching to improve living conditions or practicing good communication and social skills. The individuals do not live in a Residential Home and live in their own homes. The Life Skills Coach takes the goals that the individual is trying to achieve and help them develop the skills over time.

Qualifications: 
Education: High School Diploma or GED required.
Experience: Experience in the human service or related field with persons experiencing disabilities desirable.
Reliable transportation, a valid driver's license, and proof of automobile insurance.

Compensation:
There is a $1000 sign on bonus for this position - $500 will be paid at time of hire and $500 will be paid after 6 months in the role.
Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

The Program: Soteria Vermont is a five-bed hospitalization prevention residence for individuals experiencing an early episode of psychosis who wish to avoid or minimize their exposure to medications. 
 
Soteria is a unionized program at Pathways Vermont and the Resident Support Staff role is a unionized position within Soteria Vermont.
 
The Role: The Resident Support Staff is a member of the Soteria community which focuses on essential house functions and being with residents. Resident Support Staff substitutes are scheduled as needed - looking for availability during 1st, 2nd, and 3rd shifts. Resident Support Staff are expected to respond to a minimum of one shift every eight weeks .
 
Location: In person (Burlington VT)
Schedule: Per Diem (As needed 7 Days a week; Third shift positions available)

Summary: The Asleep Overnight Direct Support Professional 1 will provide support and assistance to individuals with developmental and psychiatric disabilities during the overnight hours as needed in Level 1 residential programs. Staff will support individuals who may need daily intervention, supervision, and skills training in activities of daily living while managing within a home environment and in the community. Individuals may require moderate to considerable physical assistance or accommodations. Schedule: Friday and Sunday 10:00pm - 8:00am (11:00pm-6:00am asleep), and Saturday 10:00pm - 12:00pm (11:00pm-6:00am asleep) (24 hours per week)
Essential Duties and Responsibilities:

<li>Support and assist individuals with all activities of daily living during the morning routine.</li>
<li>Assist individuals during the overnight shift who may wake up or require assistance.</li>
<li>Provide services to individuals according to, DDS, DMH, MassHealth, HUD, DPH, CARF, all applicable regulatory standards, and Bridgewell policies and procedures.</li>
<li>Maintain daily and monthly progress notes, shift checklists, and other program documentation according to DDS, DMH, and all applicable regulatory standards.</li>
<li>Complete household cleaning duties as assigned.</li>
<li>Maintain safety of all individuals while on shift including implementing emergency policies as needed.</li>
<li>Administer medications according to MAP regulations.</li>
<li>Maintain core training certifications including CPR/FA, OSHA, CPI, PABC, and MAP if applicable.</li>

Required Education/Experience:
